Changes Of 18F-FDG Uptake In Liver,Mediastinal Cisterna And Tumor During Chemotherapy For Hodgkin’s Lymphoma

Abstract/Summary:
Objective:To investigate the changes of 18F-FDG uptake in liver and mediastinal blood pool during chemotherapy for Hodgkin’s lymphoma and its relationship with clinical stage and tumor PET/CT metabolic parameters.Methods:41 patients with biopsy-proven HL were enrolled,and the 18F-FDG PET/CT(Positron Emission Tomography/Computer Tomography)data were retrospectively analyzed.All of patients underwent baseline(PET0),two ABVD cycles(PET2),four cycles(PET4)and six cycles(PET6)18F-FDG PET/CT scans.The maximum and mean standardized uptake values(SUV)of 18F-FDG in liver and mediastium,the maximum standard uptake value(SUVmax)and metabolic tumor volume(MTV)of the primary tumor,and the total glycolysis(TLG)of the lesion were calculated.They were all respectively calculated using the same standard protocol for PET0,PET2,PET4 and PET6.Results:1.The SUVmax of 18F-FDG were increased in liver of PET2(3.42±1.04)and PET4(3.44±0.60)compared with which of PET0(2.96±0.75;P<0.05 for both PET2 and PET4)and PET6(3.13±0.58;P<0.05 for both PET2 and PET4).The same results were obtained when considering liver SUVmean in PET2(2.27±0.76)and PET4(2.24±0.45)as compared with which of PET0(1.84±0.40;P all<0.05)and PET6(1.93±0.38;P all<0.05).2.No significant differences were obtained when comparing mediastinum SUVmax and SUVmeanin PET0,PET2,PET4 and PET6(P>0.05).3.Another finding was a negative correlation between liver SUVmax with stage in PET0(r=-0.637,P<0.001),while liver SUVmean,mediastinum SUVmax and mediastinum SUVmean were not related to stage(r=-0.286,-0.058,-0.004;P=0.070,0.717,0.979).4.Liver SUVmax of PET0 was not correlated with tumor SUVmax(r=0.028,P=0.861),while liver SUVmax was negatively correlated with tumor MTV(r=-0.537,P<0.001)and tumor TLG(r=-0.359,P=0.021).There was no correlation between liver SUVmean and tumor SUVmax(r=-0.062,P=0.699),but there was negative correlation between liver SUVmean and tumor MTV(r=-0.419,P<0.006)and tumor TLG(r=-0.355,P=0.023).5.There was no significant correlation between the mediastinal blood pool SUVmaxof PET0 and tumor SUVmax(r=0.181,P=0.258),tumor MTV(r=0.086,P=0.594),tumor TLG(r=0.201,P=0.207).There was no significant correlation between mediastinal SUVmean and tumor SUVmax(r=0.097,P=0.547),tumor MTV(r=0.174,P=0.277),tumor TLG(r=0.248,P=0.117).6.In the treatment after two cycles,the changes of liver SUVmaxand tumor SUVmax(r=0.256,P=0.106)had no significant correlation.However,it was negatively correlated with the changes of MTV(r=-0.385,P=0.013),TLG(r=0.354,P=0.023).There was no significant correlation between liver SUVmean and tumor SUVmax(r=-0.394,P=0.025),MTV(r=-0.235,P=0.140),TLG(r=-0.273,P=0.084).Conclusion:1.The results of our study suggest that liver 18F-FDG uptake is increased in patients with HL duing the chemotherpy treatment.2.The variability of liver 18F-FDG uptake should be taken into account when which is used as a parameter to assess the Deauville score of PET/CT scan during interim chemotherpy.3.The liver SUV of PET0 was negatively correlated with MTV and TLG;The mediastinal SUV of PET0 was not associated with tumor MTV and TLG.4.In the treatment after two cycles,the changes of SUVmax in the liver were negatively correlated with the changes of MTV and TLG.There was no significant correlation between liver SUVmean and tumor SUVmax,,MTV and TLG.
