| Objective: To retrospectively analyze the changes of death causes and the death age of patients with advanced schistosomiasis(late blood)in Jiaxing,and to analyze the effect of standardized comprehensive treatment on the causes of death,life expectancy and quality of life in patients with late blood after national late blood medical assistance.Methods: The death data of 167 patients registered in our hospital from 2010 to 2020 and received national late blood medical assistance(Medical assistance group)were collected,including the causes of death,sex,age of death,etc.and compared with the data of patients who did not receive national late blood medical assistance(No medical assistance group)from 1998 to 2008 in this city.Patients who received national late blood medical assistance from 2010 to 2020 received standardized comprehensive treatment,including general treatment,nutritional support treatment,treatment of ascites,spontaneous peritonitis,hepatic encephalopathy,portal hypertension and multidisciplinary treatment of upper gastrointestinal bleeding.Results: There was no significant difference in the causes of death between males and females from 2010 to 2020(χ2=7.80,P=0.35).The causes of death included liver failure,upper gastrointestinal bleeding,hepatic encephalopathy,liver cancer,infection,other cancers,heart disease,stroke,renal failure,accidental death,unknown causes and senility.The age of death was mainly from 70 to 79 years old,and the average age of death was 74.83 years old.From 55 to 69 years old,they mainly died of upper gastrointestinal bleeding,and over 70 years old,they mainly died of liver failure.The top four causes of death of late blood patients in Jiaxing from 1998 to 2008 were upper gastrointestinal bleeding,hepatic encephalopathy,unknown cause and liver failure;the age of death was mainly from 60 to 69 years old,and the average age of death was69.30.Compared with the data from 1998 to 2008,the death cause and age composition of late blood patients in 2010-2020 changed,and the difference was statistically significant(χ2=63.42,P=0.00;χ2=50.09,P=0.00).Conclusion: The patients without standardized comprehensive treatment mainly died of upper gastrointestinal bleeding,and the patients after standardized comprehensive treatment mainly died of liver failure,and the causes of death of late blood patients in Jiaxing changed significantly;standardized comprehensive treatment can reduce the incidence of upper gastrointestinal bleeding in patients with cirrhosis,prolong patient life expectancy,improve patient quality of life and is worth clinical popularization and application. |
