ObjectiveTranslating the patient-centred outcome questionnaire,to provide tool for the assessment of treatment expectations in patients with chronic musculoskeletal pain.To understand the current status of treatment expectations in patients with chronic musculoskeletal pain and analyze its influencing factors, aiming to improve the understanding of patients and medical staff on treatment expectations, provide reference for meeting or adjusting patients’ treatment expectations, and selecting the best treatment plan;in order to reduce the gap between patients’ expectations,to provide the basis for improving patient outcomes and satisfaction.Methods1.The Brislin translation model of positive translation, back translation, crosscultural adjustment and pre-investigation was used to localize the questionnaire, and the reliability and validity of the questionnaire were evaluated by calculating the testretest reliability, internal consistency reliability and content validity.2.A cross-sectional survey of 403 inpatients with chronic musculoskeletal pain in 2 tertiary A-level hospitals in Lanzhou City from June to December 2021 was selected as the research subjects;Chinese version of the patient-centered outcome questionnaire, hospital anxiety and depression scale, fear avoidance beliefs questionnaire, pain catastrophization scale, general self-efficacy scale, barthel index scale and toronto alexithymia scale were selected as research tools.SPSS 25.0 was used for statistical analysis of the data, statistical descriptions are made using frequency, rate, composition ratio,(mean ± standard deviation)or median and interquartile spacing;the univariate analysis mainly used independent sample t test,one-way analysis of variance, nonparametric test and Pearson correlation analysis, and the multivariate analysis mainly used multiple linear regression analysis.Results1.The Chinese version of the patient-centered outcome questionnaire has a total of 5 items,Test-retest reliability was 0.897,Cronbach’s alpha coefficient is 0.829,SCVI/UA and S-CVI/Ave are 0.80 and 0.97.2.(1)Patients with chronic musculoskeletal pain had an average score of usual level pain is(6.79±1.94),fatigue is(3.93±3.02),emotional distress is(3.51±2.79),interference with daily activities is(4.37±2.91);(2)The degree of reduction of desired levels symptom is: pain 91.3%,fatigue 87.3%,emotional distress 84.6%,interference with daily activities 90.6%;(3)The degree of successful levels symptom reduction is:pain 66.1%,fatigue 60.6%,emotional distress57.8%,interference with daily activities66.4%;(4)The degree of expected levels symptom reduction is: pain 68.6%,fatigue62.1%,emotional distress 59.3%,interference with daily activities 68%;(5)The mean important levels of pain is(9.40±1.12),fatigue is(6.19±4.00),emotional distress is(5.73±3.69),and interference with daily activities is(8.08±3.20).3.(1)Patients with chronic musculoskeletal pain have the main influencing factors for usual level of pain are age, pain nature,fear-avoidance beliefs, pain catastrophic and pain location, the main influencing factors for fatigue are ADL score, anxiety and disease diagnosis, the main influencing factors for emotional distress are anxiety and depression, the main influencing factors of interference with daily activities are gender,ADL score, pain catastrophic and fear avoidance beliefs;(2)The main influencing factors of desired levels pain are pain catastrophic, number of previous hospitalizations and ADL score, the main influence of fatigue the factors are pain catastrophic and anxiety, the main influencing factors of emotional distress are occupation, anxiety and pain catastrophic, the main influencing factors of interference with daily activities are pain catastrophic,ADL score and pain time;(3)The main influencing factors of successful levels pain it is the number of previous hospitalizations and the location of pain, the main influencing factors of fatigue are pain catastrophic and general self-efficacy,the main influencing factors of emotional distress are anxiety, fear avoidance beliefs and general self-efficacy,and the main influencing factors of interference with daily activities are age,Pain catastrophic and fear-avoidance beliefs;(4)The main influencing factors for expected levels of pain are the number of disease information sources, pain catastrophic, educational level and ADL score, the main influencing factors for fatigue are pain catastrophic and gender, and the main influencing factors for emotional distress are depression,anxiety,fear-avoidance beliefs, number of disease information sources, general self-efficacy,the main influencing factors of interference with daily activities are pain catastrophic,ADL score, and fear-avoidance beliefs;(5)The main influencing factors for the importance of pain symptom improvement are age, alexithymia and types of medical expenses, the main influencing factors of the importance of improvement in fatigue are anxiety,ADL score, disease diagnosis and pain location, the main influencing factors of the importance of improvement in emotional distress are anxiety, depression and educational level, the improvement of interference with daily activities the main influencing factors for importance are fear avoidance beliefs,ADL score, and number of previous hospitalizations.Conclusions1.The Chinese version of the patient-centered outcome questionnaire has good reliability and validity, and can effectively measure the level of treatment expectations of patients with chronic musculoskeletal pain in China.2.Patients with chronic musculoskeletal pain were in moderate pain and interference with daily activities, mild fatigue and emotional distress in the past week, and their desired expectations, successful and expected expectations after treatment were generally higher than expectations for clinical symptom relief, improvement in pain and limited daily activities in patients is highly important, and improvement in fatigue and emotional distress is moderately important.3.Treatment expectations of chronic musculoskeletal patients vary by disease, demographic, and psychosocial characteristics, suggesting that patients with different characteristics should be considered to develop a personalized diagnosis and treatment plan that can meet their treatment expectations, suggesting that intervention in patients with unrealistic treatment expectations should start with evaluating and intervening in their disease characteristics or psychosocial variables.
