| objective:To study the long-term efficacy and complications of iris cerclage in the treatment of mydriasisMethods:This is a retrospective self-control study.From January 2016 to June 2020,17 patients underwent iris cerclage due to mydriasis were recruited.The pupil area,visual acuity,and photophobia index of the operation eye were evaluated and matched with the postoperative condition.The relationship between changes in photophobia index and pupil size were discussed.Results:The area of the pupils which underwent iris cerclage were measured.The pupil areas were improved from(8.09±2.24)mm~2to(11.65±3.48)mm~2,P<0.01.The photophobia index were improved from(1.24±0.44)to(1.94±0.43),P<0.01.The correlation index of photophobia index and pupil area in 34 eyes r=0.875,P<0.01,which means a strong correlation between photophobia index and pupil area.Two cases of suture line cutting off the iris and one case of coil completely falling off were observed.Conclusion:The pupil area and photophobia index could rebound after the iris cerclage.The photophobia index was related to pupil areas after surgery.The suture line could cut off the iris and even completely fell off.Object:The aim of this study is to perform the iris cerclage operation on the animal mydriasis model and observe the progress of pupil area and coil movement.Methods: 6 healthy rhesus monkeys and 5 Japanese white rabbits were selected.4 monkeys were randomly divided into Experimental Group A(EGA),the other 2 monkeys were in Control Group C(CGC).The 5 rabbits were in Experimental Group B(EGB).Each eye of all the animals will be applied with the iris cerclage technique,and the operation was performed near the pupil margin about 1mm to control the pupil diameter at about 3mm.The EGA and EGB will be applied with Atropine ointment 0.2ml every 3 days to sustain the mydriatic state while the CGC applied no treatment.Photos will be taken at 1st、3rd、7th、30th、60th days with Slit lamp photography of anterior segmen to observe the progress of pupil area and coil movement.Results:1)The suture lines in EGA(8eyes)were cutting off the iris,and no coil fell off was observed 2)The suture lines in EGB(10eyes)were cutting off the iris and the coils completely fell off were observed in 4 eyes.3)The pupil areas of EGA、EGB(18eyes)were expanding,and there was significant difference between preoperative(14.64±1.11 mm~2)and postoperative(19.28±3.39mm~2)by paired-samples T test;There was no significant difference between EGA(14.77±1.27 mm~2)and EGB(14.55±1.02mm~2)by One-sample t test at 3rd day postoperatively;There was significant difference between 3-60-difference of EGA(2.05±0.922mm~2)and 3-60-difference of EGB(6.71±2.92mm~2)by One-sample t test.4)The pupil area and suture line of CGC(4eyes)do not show any variation.Conclusion:1)In the mydriatic model,the suture line of iris cerclage would erode and cut off the iris organization,and it may result in the coil falling off 2)In the mydriatic model,the pupil size still remains a tendency of expanding after iris cerclage operation.3)In the mydriatic model,iris of rabbit was more likely to be eroded by suture line than iris of rhesus. |
