| Purpose:To investigate the safety and effectiveness of Bletilla striata in the treatment of artificial ulcer after endoscopic gastric mucosal dissection.Material and method:In this study,48 patients with gastric mucosal lesions or submucosal lesions who received endoscopic mucosal dissection in the gastroscopy department of the General Hospital of the Northern Theater Command of the People’s Liberation Army from April 2020 to November 2020 were selected and screened according to the inclusion riteria and exclusion criteria.They were randomly divided into control group(n = 24)and study group(n = 24).The control group was treated with esomeprazole alone,and the study group was treated with Bletilla striata alone combined with esomeprazole.Comparison of two groups of patients with clinical data,pathological changes,operation time,initial artificial ulcer area,length of hospital stay,postoperative hospitalization expenses,and after 4 weeks of healing of ulcer area,symptom integral of 4 weeks after operation,the postoperative complications,preoperative and postoperative liver result of 4 weeks(ALT and AST),kidney(CR and BUN)and postoperative pathological diagnosis,and statistical analysis.Results:1.There were no statistically significant differences between the two groups in terms of general clinical data and postoperative pathology,such as age,sex composition,lesion site,initial artificial ulcer area,duration of operation,length of postoperative hospital stay and hospitalization cost(P>0.05).2.The area of artificial ulcer 4 weeks after ESD was compared between 2 groups(1.73±1.24 VS 3.73±2.75cm),and the difference was statistically significant(P<0.05).The healing of artificial ulcer at 4 weeks after gastric ESD was compared between the two groups.The healing rate and improvement rate of the experimental group were 66.67% and 91.67%,while the healing rate and improvement rate of the control group were 29.17% and 75.0%.The healing rate of the experimental group was significantly higher than that of the control group,with statistical difference(P<0.05).3.Symptom scores were compared between the two groups 4 weeks after surgery.Symptom scores decreased in both groups,but were significantly higher in the study group than in the control group,including the aspects of epigastrium puffmantis(0.76±0.33 VS 1.67±1.13),short yellow urine(1.00±0.37 VS 1.92±0.10),and proxis(0.41±0.08 VS 1.42±1.10),with statistical differences(P<0.05).The clinical symptom efficacy indexes of the two groups were com pared.In the study group,11 cases were cured,9 cases were significantly effective,4 cases were effective,and 0 cases were ineffective.In the control group,1 case was cured,2 cases showed obvious effect,20 cases were effective,and 1 case was ineffective.The difference was statistically significant(P<0.05).4.There were no significant differences in liver function(ALT and AST)and kidney function(CR and BUN)between the two groups before and 4 weeks after surgery(P>0.05).5.In terms of complications,there was one case of delayed bleeding within 24 h after ESD in each group,with no statistically significant difference(P>0.05).Conclusion:1.Endoscopic mucosal dissection is very safe and effective in the treatment of gastric mucosa and submucosal lesions.2.The efficacy of traditional Chinese medicine Bletilla striata combined with esomeprazole in the treatment of artificial ulcer after gastric intravenous chemotherapy is better than that of esomeprazole alone,which can accelerate the healing speed of artificial ulcer after gastric intravenous chemotherapy,improve the healing quality of ulcer,and improve the clinical symptoms of patients with good safety.3.The incidence of intraoperative bleeding from the cardia was higher than that from other parts of the stomach. |
