Exploration On The Medication Rule Of Traditional Chinese Medicine External Application Fumigation And Washing Method In The Treatment Of Gouty Arthritis

Purpose: This topic collects and summarizes(January 1991-December 2020)Chinese websites including How Net,Wanfang,Weipu,Duxiu and other Chinese websites contain journals related to the treatment of gouty arthritis with external application of Chinese medicine,using frequency analysis,Cluster analysis,association rules and factor analysis.It reveals the rule of medication for the treatment of gouty arthritis by the method of fumigating and washing with external application of traditional Chinese medicine.Material and method: The inclusion criteria and exclusion criteria were formulated,and the "prescription database of TCM External Application fumigation and washing method for treating gouty arthritis " was established.Input the serial number and traditional Chinese medicine information into Microsoft Excel 2016 one by one,use IBM SPSS 18.0 software for frequency analysis and cluster analysis;use IBM SPSS modeler18.0 software for association rule analysis,and analyze and process the collected data.Results:1.Frequency analysis: according to the inclusion criteria and exclusion criteria,245 prescriptions and 238 medicines were selected for statistical analysis.Among them,the Chinese medicine with frequency of more than 15 times is high-frequency Chinese medicine,with a cumulative frequency of more than 60%,and a total of 37 kinds.The top 10 drugs with higher frequency are: cork,rhubarb,atractylodes,borneol,gardenia,coptis,myrrh,angelica,frankincense,safflower.2.Nature and flavor return analysis: statistics of all 238 kinds of traditional Chinese medicines for medicinal properties,medicinal taste,and meridian analysis,the results show that: the medicinal properties are mainly cold,warm,and flat,accounting for 93.34%;medicinal taste is the most bitter,with a frequency of 42.45% and pungent taste 30.66%,18.24% sweet.The top 3 drugs in menstruation were liver,spleen,and stomach,accounting for 50.39% of the total.3.Association rule analysis: For drugs with a drug frequency greater than 15 times,the second-order,third-order,and fourth-order association rules were analyzed respectively.The second-order association rules were the most supported by rhubarb and cork,with a support rate of 63.673%;Rhubarb and Frankincense+Myrrh in the highest-level association rules are supported by 19.593%;in the fourth-order association rules,Gardenia and Coptis+Rhubarb+Phellodendron are supported by 14.694%.Factor analysis: Perform factor analysis on 37 high-frequency Chinese medicines with a frequency greater than 15 and extract them into 13 components.4.Cluster analysis: cluster analysis of drugs with frequency ≥15 times to obtain 4 cluster combinations.Class 1: Myrrh,Frankincense,Herbaceous Aconitum,Chuanwu,Asarum,Angelica,Guizhi,Smilax,Pittosporum bark,Safflower,Peach Kernel,Chuanxiong,Weilingxian,Tougucao;Class 2: Trichosanthes,Magnolia officinalis,Dannanxing,Angelica dahurica,turmeric,licorice,Achyranthes bidentata,Coix seed,Alisma,Atractylodes,Lonicera japonica,Red peony root,Corydalis;Class 3: Phellodendron,Rhubarb,Coptis,Scutellaria,Gardenia,Borneol,Polygonum cuspidatum;Class 4: Peppermint,Gypsum,Eupatorium.Conclusion:1.According to the study of the frequency,nature and meridian of traditional Chinese medicine,the "external application fumigation and washing method" is mainly used to clear heat and remove dampness,promote blood circulation and remove blood stasis in the treatment of gouty arthritis,and less is used to strengthen the strength and replenish deficiency,among which dampness-heat accumulation and dampness-heat stasis are the most common syndromic types.2.The association rule analysis showed that the combination of Rhubarb,Phelodendron phellodendron,Fructus gardenia and Atractyloides Atractyloides was stable,and the connection between frankincense and myrrh was strong.It could clear heat and relieve dampness,purge fire and detoxify,promote blood circulation and relieve pain,indicating that the combination of various drugs was more commonly used to treat the syndrome of dampness and heat accumulation.3.By cluster analysis,4 kinds of cluster combinations were obtained,including 2 basic prescriptions and 2 groups of addition and subtraction groups.Class 3 medicine composition is Zhi Huang Zhitong powder foundation prescription;Category 2 is the addition and subtraction of the basic formula of Simiao San,while Category 1 is the basic formula of Zhuyu San,which is used to promote blood circulation,remove blood stasis and relieve pain.It should be appropriately supplemented and subtraction on the basis of the basic formula in combination with clinical symptoms.
