Objective:Traditional Chinese medicine injection is widely used in adjuvant therapy of malignant tumors.Its combination with platinum containing dual drug chemotherapy is a common clinical treatment scheme.According to previous studies,traditional Chinese medicine injections can improve the quality of life and reduce adverse reactions in the treatment of advanced non-small cell lung cancer.However,there is no study to clarify the difference between the synergistic and toxic effects of traditional Chinese medicine injections alone or in combination on chemotherapy,which can provide a basis for the rational clinical application of traditional Chinese medicine injections.The purpose of this study is to explore the impact of single(one)or combined(two)use of traditional Chinese medicine injection on the clinical efficacy of dual drug platinum containing chemotherapy,including effectiveness and safety,and objectively analyze its rationality and practical value.Methods:100 patients with stage ?B-? non-small cell lung cancer who were hospitalized in the oncology department of Beijing Hospital of Traditional Chinese Medicine from January 1,2015 to December 31,2019 and received platinum containing dual drug chemotherapy for ? 2 cycles were included in the study.Among them,21 patients without traditional Chinese medicine injection were included in the blank group,47 patients with single traditional Chinese medicine injection were included in the observation group,and 32 patients with two traditional Chinese medicine injections were included in the combined group.The main observation indexes were progression free survival(PFS).Age,gender,pathological type,lesion metastasis,traditional Chinese medicine symptoms,traditional Chinese medicine tongue and pulse Safety indicators,functional assessment of cancer therapy-lung(fact-L)score,Karnofsky Performance Scale(KPS),such as demographic(gender,age)and oncological data(pathological type...)and safety indicators.Cox multivariate analysis regression model was constructed after single factor screening,It was verified by propensity score regression adjustment(PSRA).Results:a total of 58 males and 42 females were enrolled.The age of the enrolled patients was 34-84 years old,including 65 cases of first-line chemotherapy,35 cases of non-first-line chemotherapy,73 cases of adenocarcinoma,19 cases of squamous cell carcinoma and 6 cases of other types of non-small cell lung cancer.The minimum diameter of lung target lesions was 5mm and the maximum was 134mm.In the baseline analysis,there were differences in age distribution among the groups(P=0.029).Univariate analysis showed that the evaluation effect after 2 cycles of chemotherapy was PD,pulse was yin or yin and Yang,TCM syndrome differentiation was empirical(eight principles syndrome differentiation-deficiency and excess)and blood stasis syndrome(Qi,blood and body fluid syndrome differentiation)were the independent risk factors affecting PFS(P?0.05),while the treatment scheme(single or combined application of traditional Chinese medicine injection),age,gender,KPS score,pathological type,lung target lesions There was no significant difference in the influence of factors such as FACT-L score on PFS.Cox regression showed that Yin pulse(including sink,astringent,weak,string,fine,late and micro)and Yin and Yang combined pulse(including fine slip,sink slip,fine number,floating weak,etc.)were independent risk factors for PFS in patients with advanced NSCLC(P?0.01).PSM showed that the protective tendency of traditional Chinese medicine injection on PFS:combined group>single drug group>blank group(P>0.05).There was no significant difference in quality of life evaluation(FACT-L scale)and safety indexes among the groups,suggesting that the safety of traditional Chinese medicine injection alone or in combination was good.Conclusion:On the basis of platinum containing dual drug chemotherapy,the application of traditional Chinese medicine injection has the trend of prolonging PFS in patients with advanced NSCLC.The combination of attack and supplement may have the advantage of delaying tumor progression compared with single traditional Chinese medicine injection;The pulse of yin and Yang is common in patients with advanced non-small cell lung cancer.Traditional Chinese medicine pulse has unique application value in clinical treatment and prognosis of malignant tumors;The safety of traditional Chinese medicine injection is good.
