Depression is one of the most common mental disorders,the prevalence of women is about 2 times that of men,and the incidence of perimenopausal women is about 2 times the other period of women.It can be seen that perimenopausal women are more likely to suffer from depressive disorder,which may be due to this stage of special hormonal changes,as well as social competition,life stress and other reasons.At the same time accompanied by hot flashes,palpitation,pain and other physical symptoms,not timely treatment often delayed healing,the quality of life had a serious impact,so the pathogenesis of depression and treatment methods are imminent.With the development of new generation imaging techniques such as diffusion tensor imaging in recent years,it has been possible to analyze changes in brain functional activity and white matter fibers in depressive patients.Research on the imaging of brain function in depression has been studied worldwide to carry out quickly.OBJECTIVE:To analyze the first incidence of untreated perimenopausal period by using diffusion-tensor imaging(DTI)technique and using tract-based spatial statistics(TBSS)based on voxel analysis.Objective To investigate the pathologic mechanism of brain function in patients with MDD from the perspective of neuroimaging and to explore the correlation between the brain injury index and the severity of the disease and the course of the disease.METHODS:The "DSM-V" diagnostic tool was used to screen the diagnostic criteria consistent with the major depressive disorder without co-morbidity,aged 45-55 years old,never received clinical treatment,30 patients with first episode perimenopausal depression.20 patients(normal control group)were enrolled in the normal control group according to their age and educational level.The QIDS score was evaluated by QIDS-16.The QIDS score of the patients was≥12,and the normal control group QIDS score<6 points.Subjects were enrolled in the brain after the conventional structure of the MRI scan,excluding brain encephalopathy,etc.,followed by DTI scan.23 cases of good image data and 18 cases of normal control were studied by imaging and other factors.After the data pretreatment with PSL and other software,the mean diffusivity(MD)between the two groups was compared with the tractive-based spatial statistics(TBSS)method based on voxel analysis.Radial diffusion(RD),with P<0.05 as the difference between the two groups was statistically significant area.The difference between the statistical significance of the brain area superimposed on the standard depressive menopausal women with deprivation were compared with the normal control group.The patients with depression had the posterior part of the corpus callosum,the external capsule,the fornix,the posterior radiograph,the posterior capsule,(P<0.005)in the area of the corpus callosum,the upper part of the radiation crown,the upper beam,the sagittal layer(including the lower longitudinal bundle and the lower part of the pillow)area were significantly lower than those in the normal control group(P<0.005)The RD value of white matter in the posterior part of the lens,the upper part of the radiation crown,the longitudinal beam and the sagittal layer was significantly lower than that of the normal control group(P<0.005).Spearman’s correlation analysis showed that the absolute value of MD in the left corpus callosum body and intracystic lens was significantly negatively correlated with SSS(P<0.05),and the absolute value of RD in the left posterior capsule was similar to that of SSS There is a significant negative correlation.There was a significant positive correlation between the absolute value of MD in the left posterior radiograph and the QIDS score,and the remaining absolute values were not significantly correlated with the clinical scale scores(P>0.05).In addition,the mean duration of the patients was There was a positive correlation between the absolute value of MD in the left corpus callosum and external sac,and there was no significant correlation with the absolute value of MD and RD in other brain regions(P>0.05).CONCLUSION:1.This study found that the first untreated perimenopausal MDD patients with left lateral corpus callosum body,the left corpus callosum body and the left side of the radiation crown and other MD,RD abnormalities,suggesting that MDD patients with extensive brain White matter microstructure damage,and speculation that white matter lesions may have existed early in the onset of depression.2.The increase in MD and RD in the left BCC of the MDD patient group may suggest a lateralization of the function,and the degree of absolute increase is related to the severity of the disease.
