Objective: To explore the related risk factors of acute myocardial infarction(AMI)complicated with hypotension and clinical dialectical analysis in order to prevent the occurrence of risk factors as soon as possible during the development of the disease or eliminate as soon as possible after the occurrence of risk factors,Help to improve the prognosis,prolong the life of patients and improve the survival rate of patients.Methods: Adopts case-control study method,collected a total of 111 acute myocardial infarction patients,of which 41 cases of acute myocardial infarction complicated with hypotension,70 cases were patients with simple acute myocardial infarction without hypotension.The clinical data were collected,including laboratory data,imaging results,coronary angiography results and other related data,and use statistical knowledge to analyze the data to detect the relevant indicators from patients with acute myocardial infarction complicated with hypotensionResults:(1)The results of this study showed that the indexes of inflammation such as leukocyte and neutrophil in patients with acute myocardial infarction all increased to some extent,but the index of inflammation in acute myocardial infarction with hypotension was more obvious(P <0.05).(2)D-dimer and fibrinogen were elevated in patients with AMI,while patients with AMI with low-pressure were higher than those without AMI(P <0.05).(3)Logistic regression analysis showed that arrhythmia,right coronary artery infarction,complete occlusion and multiple vessel blockage were independent risk factors for post-acute myocardial infarction with hypotension.The history of pre-myocardial infarction was acute myocardial infarction Hypotensive protective factors occurred(OR = 0.238,P <0.05).(4)Qi deficiency and blood stasis were the main syndrome types(41.4% VS 58.5%,P(29)0.05)in TCM Syndrome Differentiation,but not in AMI combined with AMI without hypotension There was significant difference between the two groups(29.2% VS 10%,P <0.05).Conclusion: the degree of thrombosis and Inflammation in patients with AMI and hypotension is related to the degree of coronary artery disease and the extent of myocardial injury in patients with acute myocardial infarction combined with hypotension.Myocardial infarction patients with arrhythmia,right coronary lesions,complete occlusion,multi-vessel occlusion risk,more prone to hypotension,and history of preeclamptic heart attack is the protective factor for acute myocardial infarction hypotension.Patients with acute myocardial infarction,regardless of whether there is hypotension,qi deficiency and blood stasis is still its primary syndrome,and for patients with acute myocardial infarction combined with hypotension,qi deficiency,blood stasis,yang deficiency and blood stasis syndrome is the main clinical syndrome.
