Abstract/Summary:
Objective:To investigate the clinical efficacy and safety of acupotomy under the guidance of musculoskeletal ultrasound in the treatment of primary frozen shoulder.Methods:Sixty patients with primary frozen shoulder were enrolled and divided into observation group,control group and blank group,with 20 people in each group.The observation group underwent musculoskeletal ultrasound guided acupotomy and postoperative shoulder joint function exercise;the control group underwent acupotomy and postoperative shoulder joint function exercise;the blank group only underwent shoulder joint function exercise.The three groups of patients were compared before and after treatment,2 weeks after treatment,and 3 months after treatment to analyze the clinical efficacy of the patients before and after treatment,and the clinical efficacy was analyzed by the visual analogue scale(VAS)and the Constant-Murley shoulder function score.The pain improvement and functional changes of the shoulder joint before and after treatment were evaluated.Results:The three groups of patients were compared before and after.Two weeks after treatment,the VAS scores of the observation group and the treatment group were significantly lower than before treatment(P<0.01),and the blank group was lower than before treatment(P<0.05);3 months after treatment,The VAS score of the observation group was lower than 2 weeks before treatment(P<0.05),and there was no significant difference between the control group and the blank group and 2 weeks after treatment(P>0.05);it showed that the observation group had better long-term effects in improving shoulder pain than the control Group and blank group.Two weeks after treatment,the Constant-Murley score of the observation group and the treatment group was significantly lower than before treatment(P<0.01),and the blank group was lower than before treatment(P<0.05);3 months after treatment,the Constant-Murley score of the observation group was significantly lower It is lower than 2 weeks after treatment(P<0.01),and the control group and blank group are lower than 2 weeks after treatment(P<0.05);it indicates that the observation group has no significant difference in the change of shoulder function in the short term compared with the control group.Observe Compared with the control group and the blank group,the group has certain advantages in improving the level of shoulder joint function changes in the long term.Comparison of the three groups of patients at the same time node,2 weeks after treatment,the improvement of shoulder joint pain in the observation group was better than that of the control group(P<0.01),and the control group was better than the blank group(P<0.01);3 weeks after treatment,the shoulder joint pain of the observation group was better than that of the control group(P<0.01);The improvement of joint pain was better than that of the control group(P<0.01),and the control group was better than the blank group(P<0.01);2 weeks after treatment,the shoulder joint function change level of the observation group was better than that of the control group(P<0.01),the control group Better than the blank group(P<0.01);3 months after treatment,the shoulder joint function change level of the observation group was better than that of the control group(P<0.01),and the control group was better than the blank group(P<0.01).Three months after treatment,the effective rate of the observation group was 100%,the control group was 90%,and the blank group was 35%.The observation group had better efficacy than the control group and the blank group.Conclusion:Musculoskeletal ultrasound-guided needle-knife release combined with shoulder joint functional exercises can significantly improve the clinical efficacy of primary frozen shoulder,significantly reduce shoulder pain,improve shoulder joint function,and have better performance than simple pain point needle-knife release.The characteristics of accurate positioning,low pain and high safety are easier for patients to accept,providing another new method for determining the efficacy of patients with primary frozen shoulder.
