| Background and Purpose:Chronic pelvic pain(CPP)can cause persistent and repeated pelvic pain, and even lead to changes in the psychological state of patients.Brain-derived neurotrophic factor(BDNF)and its high affinity receptor TrkB are found to be associated with a variety of chronic pelvic pain diseases. In order to explore the correlation between BDNF,TrkB and CPP diseases and their clinical application value, the expression of BDNF and TrkB in serum of patients with common CPP in gynecology department and the expression of BDNF and TrkB in lesion tissue of CPP patients were detected.MethodsFrom March 2020 to September 2020,88 patients with endometriosis,16 with gynecological malignancies,7 with adenomyosis,15 with hysteromyoma,18 with benign ovarian tumor and 14 with cervical squamous intraepithelial neoplasia were selected from Xiamen University Zhongshan hospital. The degree of pelvic pain was evaluated by visual analogue scale(VAS).Enzyme linked immunosorbent assay(ELISA)was used to detect the expression of BDNF in serum, and immunohistochemical(IHC)was used to detect the expression of BDNF and TrkB in pathological tissues. Results:The expression level of BDNF in endometriosis Group[9.011(6.04-21.009)ng/ml]was significantly higher than that in gynecological malignant tumor group[4.535(1.477-7.296)ng/ml],ovarian benign tumor group[4.882(3.458-10.8845)]ng/ml,hysteromyoma Group[4.238(3.655-5.054)ng/ml]and cervical intraepithelial neoplasia Group[4.457(2.421-6.623)ng/ml],and the differences were statistically significant(P<0.05),There was no significant difference between the two groups[5.767(4.56-11.364))ng/ml(P>0.05).According to VAS score, the expression level of BDNF in severe CPP Group[15.919(8.866-34.33)ng/ml]was significantly higher than that in mild CPP Group[4.790(4.014-6.913)ng/ml]and moderate CPP Group[7.017(4.603-11.323)ng/ml](P<0.05).The expression level of BDNF was positively correlated with the degree of pain in CPP patients(r=0.670).Immunohistochemical results showed that BDNF and TrkB were strongly expressed in the lesions of severe CPP patients. Conclusions:The expression level of BDNF in serum of patients with endometriosis and adenomyosis was significantly increased.BDNF and TrkB were highly expressed in severe CPP. |
