With the successive implementation of new rural cooperative medical insurance and urban residents’ medical insurance,China has formed a basic medical security system covering urban and rural residents,which has guaranteed the basic medical and health service needs of urban and rural residents.However,under the basic medical insurance system that divides urban and rural areas,problems such as unbalanced treatment and fragmentation of the system have become increasingly prominent,failing to effectively alleviate the risk of poverty caused by illness or return to poverty due to illness.In view of this phenomenon,the state council issued the opinions on the integration of the basic medical insurance system for urban and rural residents in 2016,requiring the integration of the new rural cooperative medical care system and urban residence insurance system as soon as possible,and comprehensively promoting the urban and rural medical insurance system,so as to continuously improve the national health level and enhance the ability to resist the economic risks of diseases.In order to explore the health performance of the insurance,this article takes the residents’ health needs as the guide,and builds a health insurance mechanism for the insurance,laying a theoretical foundation for empirical analysis.Furthermore,based on the China Labor-force Dynamics Survey(CLDS)nationwide micro data from 2014 to 2016,the self-assessed health,physical health and mental health were used as health indicators,and the panel ordered logit model,panel logit model and concentration index were used to explore the impact of the integrated medical insurance system on residents’ health.The main research conclusions are as follows :(1)The integrated medical insurance system has significant health performance,and can play a positive role in promoting residents’ self-rated health and physiological health.(2)Using the centralized index to evaluate the health equity of the integrated urban and rural medical insurance system,the study found that the integrated urban and rural medical insurance system significantly reduced the level of income-related health inequality.(3)The panel ordered logit model is used to explore the impact of the integrated urban and rural medical insurance system on residents’ mental health.Data analysis shows that the impact on residents’ mental health is not significant.Based on the above research conclusions,this article proposes policy recommendations for optimizing the integration of urban and rural medical insurance systems: accelerating the integration of urban and rural medical insurance systems and orderly dispersing disease health risks;dynamically improving medical insurance treatment to meet the health needs of residents;improving the efficiency of medical insurance management services and optimizing smart medical insurance services;guiding residents to build a scientific outlook on health,giving full play to their subjective initiative;strengthening medical insurance to alleviate poverty,and reducing the economic risk of diseases of difficult groups.
