ObjectiveMusic therapy is a new treatment that combines psychology,music and medicine.The purpose of this study was to investigate the effects of music therapy on early rehabilitation,postoperative pain,knee joint function and sleep quality in patients with phobia after unilateral total knee arthroplasty(TKA).Method1.From August 2021 to March 2022,116 patients with senile phobia who underwent unilateral TKA for the first time were randomly divided into experimental group and control group.The control group was given routine nursing care such as postoperative functional exercise,pain management and diet cooperation,while the experimental group was given additional music therapy on the basis of routine nursing.2.After the patients’ consciousness recovered and their vital signs were stable on the first day after operation,and on the third,fifth,seventh and fourteenth day after operation,the improvement of phobia in the two groups was evaluated with TSK-11 scale,and the postoperative pain,knee joint function and sleep quality were compared between the two groups.Results1.After screening,116 patients with unilateral TKA who met the study criteria in our hospital from August 2021 to March 2022 were randomly divided into control group(n = 58)and experimental group(n = 58).Among them,there were 28 females and 30 males in the control group,aged from 51 to 83 years old,with an average of(67.18 ±7.34)years,and education level: 29 are junior high school or below,21 are technical secondary school or high school,18 are junior college or above.There were29 females and 29 males in the experimental group.In terms of age,the age span was from 53 to 80 years,with an average of(69.04±6.85)years.Education: 31 are junior high school or below,14 are technical secondary school or high school,13 are junior college or above.There was no significant difference in basic data between the two groups(P > 0.05).This study was examined and approved by the Medical Ethics Committee of our hospital.2.Comparison of phobia between the experimental group and the control group:repeated measurement analysis of variance was used to compare the change trend of phobia score at 1 day,5 days,7 days and 14 days after operation between the two groups.The results showed that there was a time effect on the score of phobia between the two groups at different time points after operation(P < 0.05),and the TSK score of the two groups decreased gradually at 1 day,5 days,7 days and 14 days after intervention,and the score in the experimental group was lower than that in the control group(P < 0.05).3.Comparison of pain intensity between the experimental group and the control group: repeated measurement analysis of variance was used to compare the change trend of operative pain(VAS)at 1 day,5 days,7 days and 14 days after surgery between the two groups.The results showed that there was a time effect in the VAS scores of the two groups at different time points after operation,and the VAS scores of the two groups at 1 day,5 days,7 days and 14 days after intervention were lower than those before intervention,and those in the experimental group were lower than those in the control group.4.Comparison of knee joint range of motion between the experimental group and the control group: repeated measurement analysis of variance was used to compare the knee joint range of motion of the two groups at 1 day,5 days,7 days and14 days after operation.The results showed that there was a time effect on the range of motion of knee joint between the two groups at different time points after operation(P < 0.05),and the range of motion of knee joint in the two groups increased at 1 day,5 days,7 days and 14 days after intervention,and the range of motion in the experimental group was higher than that in the control group(P < 0.05).5.Comparison of sleep quality between the experimental group and the control group: repeated measurement analysis of variance was used to compare the sleep quality of the two groups at 1 day,5 days,7 days and 14 days after operation.The results showed that there was a time effect in the SRSS score of sleep quality between the two groups at different time points after operation,and the sleep quality of the two groups decreased at 1 day,5 days,7 days and 14 days after intervention,and the sleep quality of the experimental group was lower than that of the control group(P < 0.05).ConclusionMusic therapy is effective in postoperative nursing of patients with unilateral TKA,which can significantly relieve postoperative phobia.,effectively relieve postoperative pain,improve the range of motion of knee joint and improve the quality of sleep.
