BackgroundHypertension is one of the most common chronic non-communicable diseases.At present,China has a heavy economic burden due to hypertension and its complications.Therefore,patients with hypertension have always been the focus of national medical and health policies.However,the treatment adherence of patients with hypertension in China is still not optimistic,which not only leads to the deterioration of the condition of many patients with hypertension,but also makes it difficult to implement the policy.Although the domestic research on treatment adherence of hypertensive patients has been relatively comprehensive,there is still a lack of relevant research on the impact of psychological factors on treatment adherence of hypertensive patients from the perspective of behavioral economics.The research on treatment adherence of hypertensive patients from behavioral economics has become an international academic frontier.Therefore,it is of great theoretical value and practical significance to introduce the perspective of behavioral economics into the study of influencing factors of treatment adherence in patients with hypertension and to conduct in-depth analysis of the behavioral characteristics of treatment adherence in patients with hypertension.ObjectiveThe overall goal of this study is to measure the impact of behavioral economics factors on adherence,and try to put forward corresponding policy suggestions to improve the treatment adherence of patients with hypertension.The specific objectives are as follows:(1)To investigate the current situation and problems of behavior variables and treatment adherence of patients with hypertension;(2)To explore and analyze the impact of behavioral economics factors on adherence;(3)Put forward policy suggestions to improve the treatment adherence of patients with hypertension.MethodsThe data of this study mainly comes from the existing literature,policy documents and field survey data.Through literature research,sort out the relevant research on treatment adherence and behavioral economics,and formulate a questionnaire combined with the research purpose and content of this paper.The effective number of hypertension patients in a hospital in Jinan was investigated by the questionnaire.Data sorting and analysis are mainly completed by R language and SPSS 23.0 software.Reliability analysis and factor analysis are used to test the reliability and validity of behavioral economics questionnaire,descriptive analysis is used to understand the relevant situation of patients with hypertension,and single factor logistic regression analysis is used to identify the relevant variables of adherence of patients with hypertension,Multivariate logistic regression analysis was used to explain the influencing factors of adherence in patients with hypertension.Results(1)The scores of the adherence of patients with hypertension were investigated:the average score of hill bone hypertension treatment adherence scale of 262 patients with hypertension was 21.07±4.44,The average scores of items in each dimension from low to high were medication(1.34±0.43),sodium intake(1.76±0.61)The outpatient adherence rate was(1.86±0.64).According to the standard that more than 80%of treatment adherence was considered to have good adherence,62.59%of patients with hypertension had good adherence(score<22),and 37.41%of patients with hypertension had poor adherence(score≥22).(2)The scores of behavioral economics questionnaire in patients with hypertension were investigated:the average score of representative bias was 2.42±0.96,the average score of default options was 4.24±0.88,the average score of social norms was 3.93±0.83,the average score of mental account was 2.44±1.15,the average score of time inconsistency preference was 3.30±1.10,the average score of risk preference was 2.36±1.49,and the average score of frame effect was 1.93±1.42.(3)The influencing factors of adherence of patients with hypertension were investigated:Age,annual family income,disease severity,self-efficacy,social norms,default options,time inconsistent preferences had statistically significant effects on treatment compliance of patients with hypertension.Conclusion and suggestionsThe adherence of urban hypertensive patients in Shandong Province is at a medium level,with poor performance in dietary behavior and outpatient adherence rate.In the social demographic characteristics and disease-related data,age,annual family income,disease severity and self-efficacy have a significant impact on the treatment adherence of hypertensive patients.In the behavioral economics variables,social norms Default options and time preference have a significant impact on the treatment adherence of patients with hypertension.Based on the results of this study,the following suggestions are put forward:(1)Provide social norm information feedback to patients with hypertension;(2)Promote patients to form fixed treatment adherence habits through economic incentives;(3)Medical institutions make follow-up appointments for patients in need by default to improve the outpatient performance rate;(4)Take targeted intervention measures according to the characteristics of patients with hypertension;...
