Background:The problem of sedentary behavior and poor sleep quality has attracted more attention in the aging trend.Sedentary behavior is easy to cause a series of adverse health outcomes, and poor sleep quality will limit people’s physical and psychological harm, and produce a huge health and economic burden. Both sedentary behavior and sleep quality deteriorated significantly with age.Existing research shows that different characteristics of sedentary behavior have different effects on health status. At present, there are abundant researches on sedentary behavior and sleep quality in foreign countries, but there are few researches on the association between sedentary behavior and sleep quality in China, and there are no studies on the impact of sedentary behavior and sleep quality among older adults in rural China. Objective:To investigate the status quo of sedentary behavior and sleep quality in rural older adults, and the changes in baseline and follow-up data.To explore the influencing factors of sleep quality in rural older people,to analyze the association between sedentary behavior and sleep quality, and to explore the influence of sedentary behavior and sleep quality in rural older adults with different characteristics. To provide theoretical basis for the formulation of measures to improve the sleep quality of the older adults, care for the health of rural older adults, improve the quality of life. Methods:The data of this study were from the 2019-2020 Shandong Rural Older Adults Health Study Cohort(SREHC)study. In 2019,stratified cluster sampling was used to conduct baseline surveys in Rushan,Qufu and Laoling of Shandong Province, and follow-up surveys were conducted in 2020.Face-to-face interviews were conducted through a self-made questionnaire, and 3239 people were included in the baseline survey.2731 respondents were followed up.Descriptive analysis was used to understand the general situation of sedentary behavior and sleep quality of the old adults in rural areas,T test and ANOVA were used to understand the difference in the detection rate of poor sleep quality in the old adults with different basic conditions, and Chi-square test was used to compare the differences between groups of sedentary behavior in the old adults with different characteristics.ANOVA was used to compare the relationship between the sedentary behavior group and the sleep quality score in each dimension.Logistic regression analysis and generalized equation regression(GEE)analysis were used to explore the relationship between sedentary behavior and sleep quality and its various dimensions, and to analyze the influence differences under different characteristics. Results:In the two surveys, the detection rates of poor sleep quality in rural older adults were 50.20%and 62.76%,respectively,with a statistically significant difference(χ2=824.35,P<0.001).The detection rate of poor sleep quality was significantly different among different social demographic characteristics, health behaviors and physical health status. The sedentary behavior of the elderly was significantly correlated with sleep quality(OR=1.52,95%CI=1.221.89),and the sedentary behavior of the old adults was significantly associated with subjective sleep quality, sleep latency, sleep efficiency and sleep duration(OR=1.43-1.60 95%CI=1.161.99).There was significant correlation between sedentary behavior and overall sleep quality, subjective sleep quality, sleep latency, sleep duration and sleep efficiency in old women(OR=1.50-1.82,95%CI=1.13-2.42),but no significant correlation in old men.The effect of sedentary behavior on sleep quality was significant in older adults with chronic diseases(OR=1.53-1.81,95%CI=1.08-2.67)and daily activity limitation(OR=1.61,95%CI=1.23-2.09),but not in normal older adults. Conclusions and suggestions:Conclusions:This study found that the overall detection rate of poor sleep quality among the rural older adults in Shandong province was high, and there were certain differences in sleep quality among the old adults with different social demographic characteristics, health behavior and physical health status.Sedentary behavior had a negative influence on sleep quality in rural older adults. In different dimensions, sedentary behavior was significantly correlated with subjective sleep quality, sleep efficiency, sleep duration and sleep latency. The negative influence of sedentary behavior and sleep quality was significant in older female, patients with chronic diseases or limited ability of daily activities, but not in male, patients without chronic diseases or normal daily activities.Suggestions:1.We should Carry out sleep health publicity and education, guide older adults in rural China to improve self-health awareness.2.We should guide rural older adults to pay attention to sedentary behavior and reduce sedentary time.We will improve the support network for the participation of all members of the government, society and families.Encourage rural older adults to participate in sports activities in the scientific and reasonable way and reduce sedentary behavior.3.To develop targeted interventions for different sleep problems,we should focus on physical activities for women, patients with chronic diseases and other vulnerable older adults to improving their sedentary behavior patterns.Health screening for rural older adults should be carried out to reduce risk factors and improve sleep quality.
