Clinical Study On Distribution And Surgical Prognosis Of Distal Radius Fracture

Objective:This study discussed the distribution characteristics and regularities of distal radius fracture(DRF)and discussed the related factors affecting the recovery of wrist function after DRF surgery by retrospective investigation,in order to guide clinical treatment.By observing the clinical effect of Shenjin tablet in the treatment of wrist function recovery after DRF,this study analyzed and evaluated the clinical significance of thier application in the postoperative period.Method:1.The clinical data of patients with DRF hospitalized in the Affiliated Hospital of Shandong University of traditional Chinese medicine from August 1,2015 to August 1,2021 were collected.We analyzed retrospectively their distribution characteristics of age,gender,disease side,injury cause,onset season,medical complications,fracture classification,osteoporosis to summary the morbidity pattern.2.We collected the general information,fracture,medical complications,treatment and imaging data of patients with DRF hospitalized and surgically treated in the Affiliated Hospital of Shandong University of traditional Chinese medicine from August 1,2015 to July 31,2020.The wrist PRWE score regarded as the final evaluation index was obtained by telephone follow-up,and the patients were divided into two groups(excellent and non-excellent)for case-control study.Using SPSS software and statistical methods,the statistically significant factors were selected by Logistic multivariate regression analysis,and the relevant factors affecting the recovery of wrist function after operation of DRF were obtained.3.A total of 70 patients with DRF hospitalized in the Affiliated Hospital of Shandong University of traditional Chinese medicine from November 2020 to August 2021 were recruited and randomly divided into control group(only functional exercise after operation)and observation group(Shenjin tablet combined with functional exercise),with 35 patients in each group.General baseline information was collected from the patients,and the differences in Gartland-Werley wrist function scores and PRWE scores between the two groups after 4,8,and 12 weeks of treatment were compared for statistical analysis.Results:1.A total of 1272 patients’ data with DRF were collected,including 468 males and804 females;The incidence rate of males is greater than that of females when they are younger than 50 years old,while the incidence rate of males is less than that of females when they are older than 50 years old;The proportion of patients with long-term smoking history and long-term drinking history were 9.75% and 8.89% respectively;In the seasonal distribution,the incidence is the most in spring and the least in winter;55.19%of the patients were on the left side;The patients with fracture caused by falling accounted for 80.50%;The patients with old fracture and open fracture accounted for 5.9%;C3fracture was the most common among 1043 patients with imaging data,accounting for58.77%.2.A total of 381 patients were included,including 261 in the excellent group and120 in the non-excellent group.Through the comparison of general baseline data,there were 12 variables with statistically significant difference among the 18 prognostic factors.Through multivariate regression analysis,six main factors were selected: age,BMI,longterm drinking history,cerebrovascular history,combined nerve injury,and whether they had taken traditional Chinese medicine of promoting blood circulation and removing blood stasis during perioperative period.Gender,2MCI,long history of smoking,history of hypertension,diabetes and cardiovascular disease were 6 secondary factors.3.The general baseline data of the two groups were comparable(P > 0.05).There was no significant difference between Gartland werley wrist function score and PRWE score after the end of the 4th and 8th week of treatment,and there was significant difference between the two groups after the end of the 12 th week of treatment,and there were no serious adverse reactions in the two groups.Conclusion:DRF is a common disease in elderly women.The incidence ranged from different age groups.Its incidence is related to falling injury,osteoporosis and other factors;The patients with middle-aged or old age,overweight and obesity in BMI,long-term drinking history,cerebrovascular history and preoperative nerve injury should actively carry out wrist function rehabilitation training after operation to prevent the occurrence of poor prognosis;Shenjin tablet combined with functional exercise can effectively promote the recovery of wrist function after DRF surgery.
