Clinical Observation Of Gubao Capsule In Treating Early Alcohol Induced-osteonecrosis Of Femoral Head Necrosis Of Liver And Kidney Deficiency

Objective: To observe the clinical efficacy of Gubao Capsule in the treatment of early Alcohol induced-osteonecrosis of femoral head necrosis of liver and kidney deficiency type,and evaluate its safety and clinical application value.Provide clinical evidence and data support.Methods: From January 2020 to December 2021,68 outpatients of joint orthopedics who met the diagnostic criteria and inclusion criteria of AIONFH were randomly divided into two groups,including 34 cases in the treatment group and 34 cases in the control group.The treatment group was treated with Gubao capsules and alendronate sodium tablets,while the control group was only treated with alendronate sodium tablets.At the beginning of treatment,re-examination or follow-up after 3 months,6 months,and 9 months of treatment,Harris hip score,VAS pain score,quality of life(SF-36)score and other items were collected and counted,and the hip was reviewed.Imaging tests such as joint X-rays or MRI.Finally,the data between the two groups were compared to evaluate the clinical efficacy of Gubao Capsule in the treatment of early alcoholic femoral head necrosis of liver and kidney deficiency type.Results:1.Harris hip score showed that there was no statistical difference between the two groups before treatment(P>0.05),and there were statistical differences between the treatment group and the control group after 3 months,6 months and 9 months of treatment(P<0.001).2.The pain VAS score showed no statistical difference between the two groups before treatment(P>0.05),and there were statistical differences between the treatment group and the control group after 3 months,6 months and 9 months of treatment(P<0.001).3.The quality of life(SF-36)score showed that the difference between the two groups before treatment and after 3 months of treatment was not statistically significant(P>0.05),and the differences were statistically significant after 6 months of treatment and 9 months after treatment(P<0.05).4.The comparison of imaging manifestations showed that there was no significant difference in the number of cases of deterioration,stability and improvement between the two groups at the 3rd month and the 9th month(P>0.05),but the number of cases of deterioration,stability and improvement between the two groups at the 6th month There was a statistical difference(P<0.05).5.The intra-group comparison showed that the improvement of pain VAS score and quality of life(SF-36)score of ARCO stage I and ARCO stage II patients in the treatment group was statistically different(P<0.05).The correlation analysis between the course of disease and the curative effect showed that the course of disease was negatively correlated with the change of hip Harris score and the improvement of quality of life(rs=-0.43,P<0.05;rs=-0.92,P<0.001),and was positively correlated with the change of pain VAS score(rs=0.53,P<0.01).Conclusion:1.Gubao Capsule can improve the activity of the hip joint,reduce the pain degree of the patient,and improve the quality of life of the patient.It has good curative effect on patients with early alcoholic femoral head necrosis of liver and kidney deficiency type,and is worthy of clinical application and promotion.2.Gubao Capsules has a better effect on ARCO stage I patients than ARCO stage II patients.It has a better effect on improving hip joint function and quality of life in patients with a short course of disease,and has a better effect on improving pain in patients with a long course of disease.
