| ObjectiveGuided by the integration theory of health behavior change,this study constructed the pulmonary rehabilitation exercise nursing intervention program for patients with chronic obstructive pulmonary disease,and discussed the impact of the intervention program on patients with chronic obstructive pulmonary disease.MethodsAccording to the criteria for inclusion and exclusion,100 COPD patients who were hospitalized in two wards of the respiratory department of a tertiary hospital in Kaifeng City from December 2020 to October 2021 were selected,and the respiratory ward 1 was designated as the intervention group and the respiratory ward 2 as the control group,and the number was numbered according to the order of patient enrollment,with 50 cases in each group.The intervention group implemented the pulmonary rehabilitation exercise nursing intervention program for COPD patients based on the health behavior change integration theory.The content was divided into: individualized evaluation,promoting the acquisition of knowledge,strengthening health belief,the establishment of exercise behavior and social support.At the same time,the health diary was also included in the nursing intervention program as an auxiliary means for the establishment of home exercise behavior.The control group received routine nursing intervention,.Excel database is established,SPSS25.0 is used for statistical analysis of data,and the statistical methods of this study are mainly used: descriptive statistical analysis,chi-square test,rank and test,independent sample t test,repeated measurement analysis of variance.Results1.Among the 100 patients with COPD included in this study,a total of 9 patients were lost to follow-up,and a total of 91 participants completed the experiment.Of the 50 patients included in the intervention group,a total of 5 cases were ataxia,and among the 50 patients included in the control group,a total of 4 cases were lost to follow-up.The total loss-of-follow rate was 9%.The general data and disease-related data of the two groups of patients were compared,and the difference was not statistically significant(P>0.05),which was comparable.2.There was no significant difference in pulmonary function(FEV1/FVC,FEV1% pred)between the two groups before intervention(P>0.05);One month and two months after discharge,the pulmonary function score of the intervention group was significantly higher than that of the control group(P<0.05).The results of repeated measurement analysis of variance showed that there were time effects(FEV1/FVC:F=43.654,P<0.001;FEV1%pred: F=24.842,P<0.001)and inter group effects(FEV1/FVC: F=4.760,P<0.05;FEV1%pred: F=5.174,P<0.05),and there were interaction effects between time and groups(FEV1/FVC: F=121.118,P<0.001;FEV1%pred: F=48.772,P<0.001).3.There was no significant difference in 6MWD between the two groups before intervention(P >0.05);One month and two months after discharge,the 6MWD of patients in the intervention group was higher than that in the control group,and the difference was statistically significant(P < 0.05).The results of repeated measurement analysis of variance showed that there were time effects(F=170.260,P<0.05)and inter group effects(F=3.127,P<0.05),and there were interaction effects between time and groups(F=79.980,P<0.001).4.There was no significant difference in the degree of dyspnea between the two groups before intervention(P>0.05);One month and two months after discharge,the dyspnea score of the intervention group was lower than that of the control group,and the difference was statistically significant(P<0.05).The results of repeated measurement analysis of variance showed that there were time effects(F=46.806,P<0.001)and inter group effects(F=3.970,P<0.05),and there were interaction effects between time and groups(F=17.194,P<0.001).5.There was no significant difference in the total CAT score and the scores of each item in the two groups before the intervention(P>0.05);in the first month after discharge,the total CAT score and the remaining items of the intervention group were lower than those of the control group in one month after discharge,except for confidence,sleep and energy,and the difference was statistically significant(P<0.05);after 2 months of discharge,the total score and scores of each item in the intervention group were lower than those in the control group except for sleep,and the difference was statistically significant(P<0.05).The results of repeated measurement analysis of variance showed that there were time effects(F=86.191,P<0.001)and inter group effects(F=3.853,P<0.05)in the total cat scores,and there were interaction effects between time and groups(F=15.237,P<0.001).Conclusion:Exercise nursing intervention program for pulmonary rehabilitation of COPD patients based on the integrated theory of health behavior change can help patients establish exercise behavior,improve lung function,exercise endurance,improve the degree of dyspnea of patients,improve the quality of life of patients. |
