| Objective: To investigate the occurrence and prognosis of hyperphosphatemia in hemodialysis(hd)patients in Jiujiang area,and to explore the influencing factors of hyperphosphatemia in HD patients,so as to provide reference basis for the rational formulation of the prevention and treatment plan of hyperphosphatemia in HD patients.Methods: Jiujiang first people’s Hospital,Jiujiang Hospital of traditional Chinese medicine,Affiliated Hospital of Jiujiang University,Jiujiang Sixth People’s Hospital,Jiujiang Third People’s Hospital,Chaisang District People’s Hospital,duchangxiang people’s Hospital,Duchang Hospital of traditional Chinese medicine,Hukou people’s Hospital,Pengze people’s Hospital,Xiushui first people’s Hospital Xiushui Hospital of traditional Chinese medicine,Xiushui zhajin town hospital,Xiushui bailing town hospital,Wuning people’s Hospital,Wuning Hospital of traditional Chinese medicine,Wuning Luxi town hospital,Ruichang people’s Hospital,Ruichang Hospital of traditional Chinese medicine,Ruichang wharf town hospital,Yongxiu County People’s Hospital,Yongxiu county middle hospital,Lushan City people’s Hospital,Gongqing City People’s Hospital A total of 3300 patients with ESRD who received HD treatment in 25 medical institutions of De’an County People’s hospital were included.All patients continued HD treatment for more than 6 months.Blood phosphorus examination was performed after 6 months of HD treatment,and the occurrence of hyperphosphatemia was judged according to KDIGO guidelines.According to whether hyperphosphatemia occurred during HD treatment,the patients were divided into hyperphosphatemia group and non hyperphosphatemia group.The clinical data of all patients were investigated with the help of the his system of the patient’s Hospital,and all patients were followed up for 1 year to investigate the prognosis of the two groups.The occurrence and prognosis of hyperphosphatemia in HD patients were observed;Univariate and multivariate logistic regression analysis was used to explore the risk factors of hyperphosphatemia in HD patients.Results:(1)Among 3300 HD patients,1049(31.79%)had blood phosphorus up to standard,266(8.05%)had low blood phosphorus and 1985(60.16%)had high blood phosphorus.The incidence of hyperphosphatemia was 60.16%.(2)All patients were followed up for one year.The incidence of cardiovascular events(31.28%)and mortality(18.84%)in hyperphosphatemia group were higher than those in non hyperphosphatemia group(P<0.05).(3)In the hyperphosphatemia group,the proportion of ≥ 60 years old was higher than that in the non hyperphosphatemia group,the protein intake and phosphorus intake were higher than those in the non hyperphosphatemia group,the levels of HGB,TC and i PTH were higher than those in the non hyperphosphatemia group,the ultrafiltration coefficient of dialysis instrument,dialysis frequency and dialysis adequacy were lower than those in the non hyperphosphatemia group,and the proportion of patients using phosphorus reducing drugs was lower than that in the non hyperphosphatemia group,the difference was statistically significant(P<0.05);logistic regression analysis confirmed that protein intake,phosphorus intake,ultrafiltration coefficient of dialysis instrument,dialysis frequency,dialysis adequacy and phosphorus reducing drugs were independent influencing factors of hyperphosphatemia in HD patients(P< 0.05).Conclusion: The blood phosphorus control effect in Jiujiang region is not ideal,the incidence of hyperphosphatemia is high,and the prognosis of hyperphosphatemia patients is poor,and the incidence of cardiovascular events and mortality are very high.There were many influencing factors related to hyperphosphatemia in HD patients in Jiujiang region,and protein intake,phosphorus intake,low dialysis ultrafiltration coefficient,low dialysis frequency,inadequate dialysis,and lack of phosphorus-lowering drugs were all independent influencing factors. |
