Objective:By detecting the periodontal status and serum procalcitonin(PCT)and high-sensitivity C-reactive protein(hs-CRP)levels in patients with coronary heart disease(CHD),to explore the relationship between different periodontal status and serum PCT and hs-CRP in patients with CHD and provide new basis and ideas for the diagnosis and treatment of periodontitis and coronary heart disease Methods:From January 2020 to December 2021,139 patients who had been diagnosed with coronary heart disease and voluntarily signed the informed consent form for periodontal examination and were hospitalized in the Department of Cardiology, the Fifth Clinical Hospital of Shanxi Medical University were randomly selected as the study subjects and underwent periodontal examination according to the inclusion and exclusion criteria.According to the results of periodontal examination and the severity of periodontitis, the patients were divided into 5 groups: 15 patients with periodontally healthy CHD,25 patients with gingivitis CHD,37 patients with mild periodontitis CHD,28 patients with moderate periodontitis CHD,and 34 patients with severe periodontitis CHD.The basic clinical data, periodontal examination including plaque index(PLI),periodontal visit depth(PD),clinical attachment loss level(CAL),visit bleeding index(BI)results, serum PCT and hs-CRP of the study subjects were recorded, and the serum PCT and hs-CRP levels of patients with coronary heart disease with different periodontal conditions were compared and statistically analyzed.Results:1.Periodontal status of patients with coronary heart diseaseAmong 139 patients with coronary heart disease,15(10.80%)were periodontally healthy and 15(18.00%)had gingivitis;99(71.20%)had periodontitis, including 37(26.60%)with mild periodontitis;28(20.10%)with moderate periodontitis;and 34(24.50%)with severe periodontitis.2.Comparison of general clinical data of patients in each groupThere was no significant difference in gender, age,BMI,blood pressure and fasting blood glucose among periodontally healthy CHD,mild periodontitis CHD,moderate periodontitis CHD,and severe periodontitis CHD(P > 0.05)3.Comparison of serum PCT and hs-CRP levels in coronary heart patients with different periodontal conditionsThere were significant differences in serum PCT and hs-CRP levels between periodontally healthy patients with periodontitis and CHD patients with gingivitis(P <0.05),but there was no significant difference in serum PCT and hs-CRP levels between periodontally healthy CHD patients with gingivitis(P > 0.05)There was no significant difference in serum PCT and hs-CRP levels between periodontally healthy patients with mild periodontitis and CHD patients with gingivitis(P > 0.05);there was significant difference in serum PCT and hs-CRP levels between CHD patients with moderate periodontitis and those with gingivitis(P < 0.05),and serum PCT and hs-CRP levels were higher in CHD patients with moderate periodontitis than in those with gingivitis;there was significant difference in serum PCT and hs-CRP levels between CHD patients with moderate periodontitis and those with mild periodontitis(P < 0.05),and serum PCT and hs-CRP levels were higher in CHD patients with moderate periodontitis than in those with mild periodontitis;there was significant difference in serum PCT and hs-CRP levels between CHD patients with severe periodontitis and those with moderate periodontitis(P < 0.05),and serum PCT and hs-CRP levels were higher in CHD patients with severe periodontitis than in those with moderate periodontitis.4.Correlation analysis between serum PCT as well as hs-CRP and periodontal statusThere were positive correlations between serum PCT levels and CAL(r = 0.514,P <0.05),PD(r= 0.555,P < 0.05),and BI(r = 0.439,P < 0.05),but not statistically associated with PLI(r = 0.011,P > 0.05);there were positive correlations between hs-CRP levels and CAL(r = 0.590,P < 0.05),PD(r = 0.572,P < 0.05),and BI(r = 0.514,P < 0.05),but not statistically associated with PLI(r = 0.158,P > 0.05).5.ROC curve analysis of serum PCT as well as hs-CRP levels for chronic periodontitisThe area under the curve(AUC)of serum PCT was 0.754,Youden index was 0.366,sensitivity was 0.616,and specificity was 0.750;the AUC of hs-CRP was 0.733,Youden index was 0.385,sensitivity was 0.960,and specificity was 0.425Conclusion:There is a certain correlation between periodontal status and serum PCT and hs-CRP levels in patients with coronary heart disease, and the more severe the degree of periodontal destruction, the serum PCT and hs-CRP levels, the occurrence, development and prognosis of coronary heart disease may be related to periodontal status;serum PCT and hs-CRP levels in patients with coronary heart disease have certain reference value for the early prediction of their risk of chronic periodontitis. Therefore, this study provides clinical evidence for the study of the relationship between coronary heart disease and periodontitis and provides new ideas for the diagnosis and treatment of coronary heart disease and periodontitis... |
