Research On The Optimization Of "The Tiaoshen Changzhi Method" Acupuncture And Moxibustion Therapy For Parkinson’s Disease Tremor Based On Orthogonal Design

Objective:In this study,the method of orthogonal design was used to observe and compare the therapeutic effects of different acupuncture and moxibustion regimens on the symptoms of tremor in patients with Parkinson’s disease,so as to optimize the relatively optimal comprehensive acupuncture and moxibustion treatment plan.Methods:60 patients with Parkinson’s disease who met the inclusion criteria were randomly divided into 4 groups with 15 cases in each group.According to the method of orthogonal experimental design,acupoints（Tiaoshen Changzhi Sanliujiu acupoint selection,conventional acupoint selection）,Stimulation intensity（intensity of the three parts is different,the intensity of the three parts is the same）,the moxibustion technique（the moxibustion of the Governor Vessel heat-sensitive acupoint,the moxibustion of the Governor Vessel acupoint）,three factors and two levels are combined with the corresponding treatment plan,according to the L₄2（³）orthogonal Table arranged trials,a total of 12 weeks of treatment.The scores of the third parts of the Unified Parkinson’s Disease Rating Scale（UPDRS）,tremor scores and TCM syndrome scores were observed at the 6th and 12th weeks of treatment.The range analysis and variance analysis were performed on the scoring data of each group at different time points,and the variance analysis and t test were used for comparison between groups and within groups.Statistical analysis was performed by using SPSS 26.0 statistical software.Result:According to the comparison of MDS-UPDRS III score,tremor score and TCM tremor syndrome score before and after treatment in each group,it can be seen that after 6weeks and 12 weeks of acupuncture treatment for Parkinson’s disease tremor,all scores decreased.The difference was statistically significant（P<0.05）.After visual analysis and ANOVA analysis,it can be concluded that for the MDS-UPDRS III score,the analysis results at the 6th week and the 12th week of treatment are consistent,and both show that the most influential factor on the score is the selection of acupoints,followed by stimulation intensity,and finally moxibustion technique.In conclusion,the relatively optimal program combination is A1B1C1,that is,Tiaoshen Changzhi Sanliujiu acupoints+different intensities of the three parts+the heat-sensitive moxibustion of the Governor Vessel.For the tremor score,both the 6th week and the 12th week of treatment showed that the effect of influencing factors was A>B>C,and under the same factors,the pros and cons of the level were A1>A2,B1>B2,C1>C2,the effects were statistically significant.The preferred plan is Tiaoshen Changzhi Sanliujiu acupoints+different intensities of the three parts+the heat-sensitive moxibustion of the Governor Vessel.The most influential factor on the score of TCM senile tremor syndrome is the selection of acupoints,followed by the stimulation intensity,and finally the moxibustion technique.In the selection of acupoints,Tiaoshen Changzhi Sanliujiu acupoints are better than conventional acupoint selection;in terms of stimulation intensity,different intensities of the three parts are better than the same intensity of the three parts;in terms of moxibustion techniques,the heat-sensitive moxibustion of the Governor Vessel is better than the Governor’s meridian moxibustion.Based on the data of the 6th and 12th weeks,the relatively best combination is A1B1C1,that is,Tiaoshen Changzhi Sanliujiu acupoints+different intensities of the three parts+the heat-sensitive moxibustion of the Governor Vessel.On the whole,at the 6th and 12th week of treatment,the best solutions under different evaluation indicators were all shown as acupoint selection>stimulation intensity>moxibustion technique.The optimal combination for PD tremor is A1B1C1,that is,Tiaoshen Changzhi Sanliujiu acupoints+different intensities of the three parts+the heat-sensitive moxibustion of the Governor Vessel.It shows that Tiaoshen Changzhi Sanliujiu acupoints,different stimulation intensities of the three parts,and the heat-sensitive moxibustion of the Governor Vessel can effectively improve the tremor symptoms of PD patients.Compared with other levels of conventional acupuncture,this combination has obvious advantages.Conclusion:1.The importance of each intervention factor to the curative effect of Parkinson’s disease tremor is different.The most influential factor is the selection of acupoints,followed by the stimulation intensity,and finally the moxibustion technique.2.In contrast,the best use of acupuncture and moxibustion to treat the tremor symptoms of Parkinson’s disease is Tiaoshen Changzhi Sanliujiu acupoint selection,the stimulation intensity of the three parts is different,and the heat-sensitive moxibustion of the Governor Vessel.
