| Objective:1.Systematic evaluation and meta-analysis of the clinical efficacy of penetrative acupuncture in the treatment of foot inversion after stroke provide basis support for the feasibility of clinical randomized controlled trial protocols in this study.2.Using the design of randomized controlled trials,on the basis of the acupuncture method of "awakening the brain",the clinical efficacy and safety of yin-yang acupuncture method in the treatment of foot inversion after stroke were observed,in order to enrich the acupuncture treatment plan of foot varus after stroke.Method:1.Computer search pubmed and The Cochrane library database,CNKI full-text database(CNKI),Wanfang database(WF),Weipu full-text journal database(VIP)and Chinese biomedical literature database(CBM).Randomized control trials(RCTs)for the treatment of post-stroke foot varus using acupuncture from the library until October 31,2021,and systematically reviewed the included literature using Rev Man5.4 software.2.From October 2020 to December 2021,the participants of this experiment were selected from the inpatient department and outpatient department of the Department of Acupuncture and Moxibustion of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine,and a total of 62 patients with post-stroke foot inversion were included,and the subjects were divided into control groups and experimental groups by random number table method,with 31 cases in each group.Both groups carried out stroke-related symptomatic drug treatment,"brain awakening" main prescription,auxiliary acupuncture and routine nursing and other basic treatment,of which the control group adopted conventional acupuncture method,and the experimental group used the yin and yang penetration acupuncture method,once a day,6 times a week,for a total of four consecutive weeks.Before and after treatment,the MAS scale is used to assess the degree of muscle spasm around the ankle,the FMA scale(lower limb part)is used to evaluate lower limb motor function,the BBS scale is used to evaluate balance function,and the MBI scale is used to evaluate daily living ability.After the end of the trial,SPSS21.0 software was used to statistically analyze the relevant data to evaluate the clinical efficacy of yin-yang acupuncture in the treatment of foot inversion after stroke.Result:1.According to the criteria of 11 RCTs and 763 patients,the results showed that the experimental group(i.e.,the group with penetrating acupuncture method)was significantly better than the control group(rehabilitation treatment alone or other therapy)in improving the total effective rate and improving FMA and MAS scores.2.The comparison of MAS rating,FMA score,BBS score and MBI score in the two groups before and after treatment was statistically significant(P<0.01),indicating that both the experimental group and the control group could improve the relevant symptoms of the patients.3.The comparison of MAS rating,FMA score,BBS score and MBI score between the two groups after treatment was statistically significant(P<0.05),indicating that the efficacy of the experimental group was more significant than that of the control group.4.The experimental group(balance yin and yang acupuncture method)clinically controlled 5 cases,11 cases of apparent effect,12 cases of effectiveness,2 cases of ineffectiveness,the total effective rate of 93.3%;the control group(conventional acupuncture method)clinical control of 2 cases,8 cases of apparent effect,effective 13 cases,ineffective 7cases,the total effective rate of 76.6%,the difference is statistically significant(P<0.05),indicating that the clinical efficacy of the balance yin and yang penetration acupuncture method is better than that of conventional acupuncture method.Conclusion:1.For patients with post-stroke foot varus,the acupuncture needle treatment effect is significantly better than that of rehabilitation or other therapies alone,but due to the limited number of literature included in the study and the average quality of the literature,the study still needs to be further verified by multicentre,large sample and high-quality RCTs in the future.2.The yin-yang acupuncture method is safe and effective in the treatment of foot varus after stroke,which can significantly improve the degree of spasm,motor function,balance ability and daily living ability of patients’ lower limbs,and its overall treatment effect is better than that of conventional acupuncture therapy. |
