Measurement And Clinical Application Of Aesthetic Data Of Medial Canthus In Chinese Han Women

Background:Epicanthoplasty is a popular surgery in China.However,there are few studies on the aesthetics of medial canthus of Chinese Han women.And plastic surgeons lack accurate aesthetic data as a reference for surgery.Objective:1.The En-Lp/En-Ex differences of medial canthus were analyzed by measuring the ratio of the distance between Ectocanthion(En)and lacrimal punctum(Lp)to the distance between Ectocanthion(En)and Ectocanthion(Ex)in Chinese Han Women,providing data reference for plastic surgeons to perform epicanthoplasty.2.Based on the En-Lp/En-Ex data of different degrees of epicanthus in the first part,the epicanthus opening distance was calculated and epicanthoplasty was performed to explore the clinical significance of aesthetic data of epicanthus.Methods:1.From January 2020 to September 2020,1,052 Chinese Han women aged from 18 to 40 were randomly selected using a simple randomization method.According to the exposure degree of lacrimal caruncle,the subjects were divided into no-epicanthus group,mild epicanthus group,moderate epicanthus group and severe epicanthus group.The distance between Ectocanthion(En)and lacrimal punctum(Lp)and the distance between Ectocanthion(En)and Ectocanthion(Ex)were measured,and the ratio(En-Lp/En-Ex)was calculated.Variance Analysis(ANOVA)and Least Significant Difference(LSD)were used to calculate and analyze En-Lp/En-Ex differences of epicanthus among the four groups.The Receiver Operating Characteristic(ROC)curves for recognizing no and mild epicanthus,mild and moderate epicanthus,moderate and severe epicanthus were plotted using En-Lp/En-Ex values,and the Area Under the Curves(AUC)was calculated.EnLp/En-Ex at the maximum Yorden Index was taken as the optimal cut-off value.The sensitivity and specificity of En-Lp/En-Ex on epicanthus were analyzed.All data in this study were analyzed by SPSS 22.0 statistical software.2.From October 2020 to June 2021,47 beauty seekers undergoing epicanthoplasty in our department were selected.According to the eye conditions,aesthetic needs and EnLp/En-Ex measurements,the specific data for epicanthoplasty were calculated and the operation was performed.The results of surgery were evaluated 6 months after surgery,and patient satisfaction was counted.Results:1.A total of 1052(2104 eyes)eligible women were included in the study,including 196 eyes(9.3%)in the no-epicanthus group,590 eyes(28.0%)in the mild epicanthus group,972 eyes(46.2%)in the moderate epicanthus group,and 346 eyes(16.4%)in the severe epicanthus group.The incidence of epicanthus was 90.7%.The En-Lp/En-Ex ratio of each group:the no-epicanthus group ranged from 0.18 to 0.24,with an average of 0.21±0.03,the mild epicanthus group ranged from 0.17 to 0.21,with an average of 0.19±0.02,the moderate epicanthus ranges from 0.13 to 0.19,with an average of 0.16±0.03,and the severe epicanthus ranges from 0.10 to 0.16,with an average of 0.13±0.03.ANOVA results showed that En-Lp/En-Ex differences among the four groups were statistically significant(P<0.05),and the differences between any two groups were statistically significant(P<0.05).ROC curve results showed that the optimal cut-off value between no-epicanthus and mild epicanthus,mild and moderate epicanthus,moderate and severe epicanthus was 0.20,0.17,0.14,respectively.The ROC curve of En-Lp/En-Ex for detecting different degrees of epicanthus showed that the optimal cut-off value was 0.20(AUC=0.699,95%CI=0.657～0.740,P<0.05)for distinguishing no and mild epicanthus,the optimal cut-off value was 0.17(AUC=0.800,95%CI=0.778～0.822,P<0.05)for distinguishing mild and moderate epicanthus and 0.14(AUC=0.787,95%CI=0.760～0.814,P<0.05)for distinguishing moderate and severe epicanthus.2.In this study,a total of 47 patients were enrolled after epicanthoplasty,of whom 2 patients lost contact.6 months after surgery,the satisfactions of epicanthus and postoperative scar were 100%and 97.8%,respectively.All patients were satisfied with the form of medial canthus,and one patient(2.2%)was dissatisfied with medial canthal scarring due to swelling and redness of the medial canthus.Conclusion:En-Lp/En-Ex is an important data of eye aesthetics measurement in Chinese Han women,which has high clinical application value and can provide a reference for plastic surgeons to accurately perform epicanthoplasty.
