| Objective:Through the investigation of the general data of patients with pemphigus and concomitant diseases, this paper analyzes the intervention role of some related risk factors in the pathogenesis of pemphigus, and whether pemphigus is more likely to be complicated with some diseases,so as to increase clinicians and patients’ attention to related risk factors and concomitant diseases.More importantly,it has a certain guiding significance in the prevention, diagnosis, treatment and management of pemphigus. Methods:Patients with pemphigus who were willing to participate in the questionnaire in the outpatient and ward of our hospital from January 1,2018 to March 31,2021 were selected as the case group, and the following patients were excluded: the clinical manifestations and pathological examination were not consistent with or could not clearly diagnose pemphigus;there is a genetic relationship between the cases;suffering from other autoimmune skin diseases. In the same period, those who were not related to each other, denied having autoimmune dermatosis and were willing to participate in the questionnaire were selected as the control group. The general data and concomitant diseases of the subjects were collected by self-designed questionnaire, the tables of related data were designed by Excel software, and the relevant statistical analysis was carried out by statistical software SPSS23.0.The comparative age between the two groups was expressed by mean ± standard deviation, independent sample t-test was used, and chi-square test was used for other factors.the factors with statistical differences in univariate analysis were analyzed by multivariate binary Logistic regression model. The statistical results were statistically significant(P<0.05).Results:1.A total of 104 pemphigus patients and 143 physical examination centers were enrolled in this study. There were 54 males and 50 females in the case group, with a male to female ratio of 1.08:1.The onset age ranged from 18 to 79 years, with an average age of 51.69 years. In the control group, there were 74 males and 69 females, with a male to female ratio of 1.07:1.The onset age ranged from 18 to 75 years, with an average age of50.15 years. There were no significant differences in gender and age between the case group and the control group(P>0.05).The most common initial sites of pemphigus were oral cavity(26%),chest and back(22.1%),head and face(17.3%).2.Comparing the inducement between the case group and the control group, the single factor results showed that there were significant differences in onion and garlic vegetables, infection, mental factors, poor sleep status, exposure to pesticides and pesticides, trauma and operation, drug intake and family history between the two groups(P<0.05).There was no significant difference in smoking, drinking, horticultural materials and metal vapor(P>0.05).Multivariate Logistic regression analysis showed that vegetables eating onions and garlic, infection, mental factors, exposure to pesticides and pesticides, drug intake and family history had significant effects on the incidence of pemphigus.3.Compared with the control group, there were significant differences in autoimmune thyroid disease, rheumatoid arthritis and malignant tumor between the case group and the control group(P<0.05).There was no significant difference in systemic lupus erythematosus,Sjogren’s syndrome and type 1 diabetes(P>0.05).Multivariate Logistic regression analysis showed that patients with pemphigus were easily complicated with autoimmune thyroid disease, rheumatoid arthritis and malignant tumor.Conclusion:1.Eating onion and garlic vegetables, infection, mental factors, exposure to pesticides and pesticides, drug intake and family history play a certain role in the incidence of pemphigus, which should attract the attention of the majority of medical and health care groups, through the intervention of the above risk factors,it is expected to reduce the incidence of pemphigus or delay the development of pemphigus and reduce the risk of recurrence.2.Patients with pemphigus are easy to be complicated with autoimmune thyroid disease, rheumatoid arthritis and malignant tumor, which is helpful for clinicians to make early diagnosis and treatment of pemphigus complicated with disease. |
