A Study On Caregiver Burden Of Hospitalized Patients With Nonalcoholic Fatty Liver Disease Based On Latent Class Analysis

Objective:(1)To understand the anxiety and depression of patients hospitalized with nonalcoholic fatty liver disease(NAFLD);(2)To understand the caregiver burden level of primary caregivers of patients hospitalized with NAFLD;(3)To classify the caregiver burden using latent category analysis;(4)To analyze the factors influencing the latent category of caregiver burden and its correlation with the anxiety and depression of patients.Methods:Using a convenience sampling method,145 pairs(145 patients and 145 primary caregivers each)of patients hospitalized with NAFLD disease and their primary caregivers in a tertiary hospital in XX Province were selected as study subjects between March 2021 and August 2021 strictly according to the nadir criteria,and sociodemographic data and disease characteristics of patients and sociodemographic data and caregiving characteristics of primary caregivers were collected.The Generalized Anxiety Scale(GAD-7)and the Patient Health Depression Questionnaire Scale(PHQ-9)were used to investigate the patients’ anxiety and depression status;the caregiver burden scale(ZBI)was used to assess the caregiver burden level.SPSS 26.0was used for descriptive analysis,univariate analysis,multiple stepwise regression analysis and Pearson correlation analysis,and Mplus 8.3 software was used for potential category analysis of caregiver burden.Results:(1)NAFLD inpatients had a GAD-7 score of(6.08±5.68),with an anxiety prevalence of 48.97%;PHQ-9 score was(7.37±5.65),and the incidence of depression was 64.83%.(2)The total caregiver ZBI score was(24.85 ± 13.07),of which the personal burden dimension score was(5.89 ± 4.61)and the role burden dimension score was(12.65 ± 6.80),and the percentage of the presence of caregiver burden was62.76%.(3)Based on the conditional probability of each category,the primary caregiver caregiver burden of inpatients with NAFLD can be divided into three latent categories: low burden group(29.86%),high personal burden group(38.19%),and personal burden combined with responsibility burden group(31.94%).(4)The results of univariate analysis showed that the differences between the latent categories of caregiver burden were statistically significant(P < 0.05)for the monthly economic income of the primary caregiver,relationship with the patient,whether the patient lived with the patient,whether the patient was cared for full-time,and the length of care,and there were no statistically significant differences(P > 0.05)for the latent categories of caregiver burden for the socio-demographic information of the patient.(5)The results of the unordered multicategorical logistic regression analysis showed that low economic income and full-time caregiving patients were risk factors for the C2 group(high personal burden group).Low financial income,full-time care of patients and caregivers who were parents of patients were risk factors for group C3(combined personal burden and responsibility burden group).(6)The results of Pearson correlation analysis showed no significant correlation between patients’ anxiety and depression and the potential category of caregiver burden(P > 0.05).Conclusions:(1)High prevalence of anxiety(48.97%)and depression(64.83%)in patients hospitalized with NAFLD.(2)Caregiver burden was present in a high percentage of primary caregivers of patients hospitalized with NAFLD(62.76%).(3)Potential category analysis allowed for the classification of caregiver burden into three categories.(4)Patients’ anxiety and depression were not significantly correlated with the three potential categories of caregiver burden.Low financial income and full-time caregiving for patients are common risk factors,but C3 has its own unique influencing factors,suggesting that healthcare professionals should identify the level of caregiving burden and target interventions based on different characteristics.
