| Obgective:This study designs the scheme through orthogonal test,in order to provide an optimized treatment scheme for traditional Chinese medicine in the treatment of low back myofasciitis and provide the idea of selecting the treatment scheme for clinical treatment.Method:1 Through How Net,Wanfang and VIP,we searched the literature on the treatment of lumbar back muscular fasciitis by traditional Chinese medicine from 1986 to 2020.The key words include"lumbar back muscular fasciitis"and"low back myofascial pain syndrome".A total of518 relevant literatures were searched.After that,400 literatures meeting the requirements were selected according to the literature selection criteria,and the treatment methods used were counted,Get the basic database,analyze the theoretical basis and curative effect of its therapy,select the therapy with more accurate effect for the treatment ofdisease,and finally select acupuncture therapy and cupping therapy as factors for the test.Based on the statistics of acupuncture point selection,acupuncture manipulation and cupping methods,combined with the clinical experience of Professor Li Zhidao and director Wang Licun,Jiaji point,Ashi point and Weizhong point are finally selected as the basic points,Hegu acupuncture,side acupuncture and resistance acupuncture are selected as the three levels of acupuncture factors,and retention of cupping,cupping and flash cupping are selected as the three levels of cupping factors.2 Input the selected factors and the three levels related to the factors into the spssau website to obtain the L9(3~2)(two factors and three levels in nine groups)orthogonal test design table.Four patients in each group were included in the test.Use the same acupuncture needle and cupping during treatment,control the same factors such as needle retention and cupping time,and strictly control factors related to treatment.3 In the clinical trial stage,the principle and method of randomized control are strictly followed.The enrolled patients need to meet the diagnostic criteria of traditional Chinese and Western medicine,and ensure that the patients participate in the trial with informed consent.There were 36 patients in total,all of whom were treated in the outpatient department of our hospital from April to October 2021.Generate a group of random numbers through Excel,and divide the patients into 9 groups according to this standard.All patients were treated strictly according to the treatment scheme of their groups.All patients were treated with Jiaji point(the part corresponding to the pain area),Ashi point and Weizhong point as the basic treatment.According to their groups,they were treated with different acupuncture techniques and cupping methods,with 2 weeks as a course of treatment,3 times a week,Monday,Wednesday and Friday,with a total of 6 times in a course of treatment,Cupping was performed at the 1st,3rd and 5th times of treatment.The efficacy before and after treatment was evaluated.For pain symptoms,visual analogue scale(VAS)was used,and Oswestry dysfunction index(ODI)was used to evaluate the treatment results.After treatment,use SPSS23 0 make statistical analysis on the treatment results,evaluate the clinical efficacy of various therapies,and then get the treatment scheme with better curative effect.Result:1 Compared with before treatment,the VAS score and ODI score of 9 groups decreased significantly(P<0.05),it shows that the 9 groups of schemes can effectively reduce the pain of patients and restore their waist and back function.2 For VAS score,it can be concluded from the intuitive analysis of orthogonal test:(1)rb>r A,the analgesic effect of cupping therapy is better than acupuncture manipulation;(2)The importance of improving pain symptoms at different levels is as follows:A3>A2>A1,B2>B1>B3;That is,resistance needling is better than side needling,better than Hegu needling,walking pot is better than leaving pot,better than flashing pot;(3)The analysis of variance shows that PA>0.05 and PB>0.05,which indicates that there is no significant difference in the analgesic effect of different acupuncture techniques and different cupping treatments.The treatment scheme with better analgesic effect is resistance acupuncture+cupping.3 For ODI score,it can be concluded from the visual analysis of orthogonal test:(1)r A>r B,the effect of acupuncture on restoring low back function is better than cupping treatment;(2)The importance of improving low back function at different levels is as follows:A3>A2>A1,B1>B3>B2;That is,resistance acupuncture is better than side acupuncture and better than Hegu acupuncture;Retention tank is better than flash tank and is better than moving tank;(3)The analysis of variance shows that PA>0.05 and Pb>0.05,which indicates that there is no significant difference in the functional recovery of different acupuncture techniques and cupping treatments.Among all the schemes,the scheme with better recovery of waist and back function is:resistance acupuncture+can retention.Conclusion:1 After analyzing the VAS score,it is concluded that the scheme with the best analgesic effect is resistance acupuncture+cupping;After analyzing the ODI score,it is concluded that the best scheme to improve the quality of daily life is resistance acupuncture+can retention.2 Orthogonal test is a simple and efficient method to screen better schemes,but when it is used to optimize the treatment scheme of traditional Chinese medicine,it can not replace the doctor’s traditional Chinese medicine theory and traditional Chinese medicine thinking to guide clinical treatment,but only as a point of view to look at the relationship between different treatment methods from other angles. |
