| Objective: The relationship between acute ischemic stroke(AIS)and carotid artery stenosis(CAS)was predicted by measuring serum CRP/ALB ratio(CAR).To accumulate data and provide a theoretical basis for the study of CAS in AIS patients at medium and high altitudes.And provides an effective,reliable,simple and cheap technical means for screening and preventing high-risk groups of stroke.Methods: A total of 285 patients who were admitted to the Department of Neurology,Qinghai Provincial People’s Hospital for the first time and suffered from AIS from January 2020 to October 2021 were included in this study.Those patients who were confirmed to have no carotid stenosis according to the results of CT angiography(CTA)were classified as the control group(n=74),and those who were confirmed to have CAS were classified as the experimental group(n=211).The experimental groups were divided into three subgroups according to the degree of stenosis,namely,the mild stenosis group(stenosis rate < 50%,n = 79);Moderate stenosis group(50≤ stenosis rate ≤ 69%,n = 65);Severe stenosis and occlusion group(stenosis rate > 70%,n = 67);Blood biochemical data and baseline data(age,alcohol consumption,etc.)were collected,and the clinical data,baseline data and imaging data of the control group and the experimental group as well as each subgroup in the experimental group were compared and analyzed.Results:Compared with the control group,the difference in CAR level was statistically significant(P < 0.05).Compared with the three subgroups in the experimental group,CAR level in severe stenosis and occlusion group was higher than that in moderate stenosis group and mild stenosis group,and CAR level in moderate stenosis group was higher than that in mild stenosis group,the difference was statistically significant(P < 0.05).Ordered multiple Logistic regression analysis showed that CAR(OR=6.88,95%CI: 0.647 ~ 3.211,P < 0.001)was a risk factor for CAS in ACI patients.ROC curve showed that the optimal critical value for CAR to predict CAS in AIS patients was 0.331(area under the curve was 0.701,95%CI: 0.636 ~ 0.766,P < 0.001),with a sensitivity of 68.2% and specificity of 64.9%.Conclusions:1.CAR level in medium and high altitude areas is a risk factor for CAS severity in AIS patients,and it is better than the two inflammation markers CRP and ALB alone in predicting the CAS severity in AIS patients.2.The CAR value of CAS in AIS patients is higher than that without CAS,and the higher the CAR,the more severe the carotid artery stenosis will be;3.CAR can be used as a reliable indicator for predicting CAS in AIS patients at medium and high altitudes.At the same time,it suggests that we should pay attention to the treatment of inflammation to minimize the incidence of CAS and minimize the risk of cerebrovascular disease. |
