Objective : To detect the incidence of Small intestinal bacterial overgrowth(SIBO)in patients with colonic adenoma and colon cancer in Qinghai area by methane-hydrogen breath test,and to further explore the relationship between colonic adenoma,colon cancer and small intestinal bacterial overgrowth.Methods : A total of 39 patients with colon cancer and 45 patients with colonic adenoma were enrolled from November 2020 to December 2021 in the People ’ s Hospital of Qinghai Province.In the mean time,50 healthy subjects were selected as the control group.The general clinical data of all subjects were collected,and the questionnaire of gastrointestinal symptoms was completed.The incidence of SIBO in patients with colonic adenoma and colon cancer was detected by methane-hydrogen breath test.The positive rate of SIBO,the hydrogen and methane breath values at 6 time points were compared between the observation group and the control group.Comparison of gastrointestinal symptoms and complications between cancer group and adenoma group.According to the results of methane-hydrogen breath test,colon cancer group and colonic adenoma group were divided into SIBO(+)group and SIBO(-)group,respectively,compared with each subgroup in abdominal pain,abdominal distension,diarrhea,nausea such gastrointestinal symptoms were statistically significant.Results:1.The positive rate of SIBO in colonic adenoma group was77.8%(35/45);the positive rate of SIBO in colon cancer group was76.9%(30/39);the positive rate of SIBO in the control group was24.0%(12/50).The positive rates of SIBO in colonic adenoma group and colon cancer group were significantly higher than those in control group,and the difference is statistically significant(c2=36.546,P<0.001).There was no significant difference in the positive rate of SIBO between colon adenoma group and colon cancer group(c2=0.009,P=0.926).The values of hydrogen and methane in colonic adenoma and colon cancer group were higher than those in control group at each 6 time points.2.The incidence of diabetes in colon cancer patients was significantly higher than that in colon adenoma group,and the abdominal pain and diarrhea in colon cancer patients were significantly higher than those in colon adenoma group.3.The incidence of abdominal distension of colonic adenoma in SIBO positive group was significantly higher than that in negative group(P <0.005).The abdominal pain and abdominal distension colon cancer in the SIBO positive group was significantly higher than those in the negative group(P < 0.005).Other symptoms were not statistically significant.Conclusion : SIBO is closely related to colon adenoma and colon cancer,and is related to abdominal distension,abdominal pain,diarrhea and other symptoms of colon adenoma and colon cancer.Therefore,we believe that the occurrence of SIBO may become an early warning signal for colon cancer and colon adenoma. |
