Analysis Of Survival-related Factors In Patients With MCRPC Receiving Abiraterone Treatment

Objective: The basic information,hematological parameters,and general conditions before treatment were analyzed in mCRPC patients treated with abiraterone acetate(AA).Explore relevant parameters that can predict the prognosis of mCRPC patients treated with AA.Methods:The clinical data of 71 mCRPC patients receiving AA treatment in Affiliated Hospital of Qinghai University and Qinghai Cancer Hospital from 2017.06 to 2021.06 were retrospectively analyzed.Univariate COX regression analysis was used to analyze the correlation between patients’ prognosis and some factors,such as PSA at diagnosis,Gleason score,effective treatment time of ADT,age before the start of AA treatment,ECOG score before the start of AA treatment,RDW,NLR and Hgb before the start of AA treatment and PSA response degree after 4weeks of AA treatment.Multivariate COX regression analysis was used to analyze the previous statistically significant factors.Survival analysis was performed by Kaplan-Meier method,and significance test was performed by Log-rank method.Results: At the end of follow-up,a total of 56 mCRPC patients receiving AA treatment had progressed and 27 patients had died.Median progression-free survival(PFS)and overall survival(OS)were 12.4(95%CI:10.2-14.5)months and 27(95%CI:23.1-30.8)months.Multivariate COX regression analysis showed that PSA response degree(HR=1.829,P=0.047),ECOG score(HR=5.026,P < 0.001),and NLR(HR=1.859,P= 0.028)were independent prognostic factors for PFS in mCRPC patients treated with AA.At the same time,PSA response degree(HR=3.188,P=0.018),ECOG score(HR=3.709,P= 0.02)and NLR(HR=3.543,P=0.009)were also independent prognostic factors for OS in mCRPC patients treated with AA.ECOG score,NLR and PSA response degree were used to make K-M survival curves for PFS and OS,respectively.The results showed that patients with PSA response ≥50%,ECOG score 0,1 and NLR < 2.8 were associated with a better survival prognosis than the patients with ECOG score 2,PSA response <50% and NLR ≥ 2.8.Log-Rank P<0.05.Conclusion: PSA response degree,ECOG score,and NLR were independent prognostic factors for PFS in mCRPC patients treated with AA.At the same time,PSA response degree,ECOG score and NLR were also independent prognostic factors for OS in mCRPC patients treated with AA.
