Study On The Relationship Between Platelet Parameters And Hypothyroidism In Patients With Type 2 Diabetes Mellitus

Abstract/Summary:
Purpose:By analyzing whether there are discrepancies in platelet parameters in type 2 diabetes mellitus(T2DM)with different degrees of hypothyroidism,to study the effect and significance of platelet count(PLT),mean platelet volume(MPV),platelet volume distribution width(PDW)and large platelet cell ratio(P-LCR)in T2DM with hypothyroidism.To provide clinical reference for early diagnosis and intervention of T2DM with hypothyroidism.Methods:From October 2020 to October 2021,190 patients who were hospitalized in the Endocrinology Department of Qing Hai University Affiliated Hospital and diagnosed with T2DM were randomly selected as the observation group,and we separated the observation group into two groups on the basis of thyroid function level,T2DM combined with subclinical hypothyroidism group(96 cases)and T2DM combined with clinical hypothyroidism group(94 cases),respectively.At the same time,96 T2DM patients with normal thyroid function were collected as control group.Collect and record relevant clinical data and laboratory indicators of all study subjects.Results:(1)The general information were statistically analyzed,and there were statistical discrepancy in age and BMI among the three groups significantly(P<0.05);but there were no statistically diversity in gender and disease course among the three groups(P>0.05).(2)Relevant clinical test indicators were statistically analyzed,and there were statistical discrepancy in MPV,PDW,PLCR,LY,PT,TC,FT3,FT4,TSH and Hb A1c among the three groups significantly(P<0.05);There was no significant discrepancy in PLT,NE,NLR,PLR,TT,APTT,FIB,TG,D-HDL,DLDL,FINS,FPG,HOMA-βand HOMA-IR among the three groups(P>0.05).(3)Correlation analysis between TSH and platelet parameters among the three groups showed that TSH was positively correlated with MPV,PDW,PLCR,TC,PT,Hb A1c,age and BMI(r _S=0.376,0.257,0.304,0.519,0.128,0.497,0.222,0.175;P<0.05);TSH was negatively correlated with LY level(r _S=-0.145),and the discrepancy were all statistically significant(P<0.05).There was no correlation between PLT and TSH(P>0.05).(4)Binary Logistic regression analysis of T2DM with hypothyroidism showed that age(OR=1.115,95%CI:1.061-1.172),BMI(OR=1.218,95%CI:1.031-1.439),MPV(OR=1.604,95%CI:1.141-2.254),PLCR(OR=1.162,95%CI:1.084-1.245),TC(OR=3.539,95%CI:2.098-5.970),Hb A1c(OR=3.049,95%CI:2.054-4.526)was an independent risk factor for T2DM with hypothyroidism.(5)ROC curve was used to detect the diagnostic value of MPV,PDW and PLCR in T2DM patients with hypothyroidism.The results showed that the area under curve(AUC)of MPV was 0.739,the optimal cut-off point value of MPV for predicting T2DM with hypothyroidism was 10.85,corresponding sensitivity was 81.1%,and specificity was 56.2%.The AUC of PDW was 0.657,the optimal cut-off value of PDW for predicting T2DM with hypothyroidism was 15.85,the corresponding sensitivity was 70.5%,and the specificity was 54.2%.The area under curve(AUC)of PLCR was0.710,the optimal cut-off point value of the corresponding PLCR for predicting T2DM with hypothyroidism was 42.05,corresponding sensitivity was 56.3%,and specificity was 89.6%.Conclusion:1.MPV,PDW and PLCR in T2DM patients with hypothyroidism were significantly higher than those in T2DM patients with normal thyroid function.This suggests a possible association between platelet markers and thyroid function.2.Correlation analysis of TSH and platelet parameters among the three groups showed that TSH level was positively correlated with MPV,PDW and PLCR.This may suggest that the more severe the degree of hypothyroidism,the more significant the level of platelet indicators.3.Age,BMI,MPV,PLCR,TC and Hb A1c were independent risk factors for T2DM complicated with hypothyroidism.4.MPV,PDW and PLCR levels have certain predictive value and guiding significance for evaluating T2DM complicated with hypothyroidism.
