Objective:To explore the current situation of pain catastrophizing in patients with chronic neuropathic pain,and to comprehensively and systematically explore the influencing factors of pain catastrophizing in patients with chronic neuropathic pain from the aspects of environmental factors,previous pain perception,predisposing factors and psychological factors.Furthermore,to analyze the parallel mediation effects of psychological resilience and somatization on alexithymia and pain catastrophizing,which might reveal the complex relationships among the influencing factors of pain catastrophizing in patients with chronic neuropathic pain,as well as lay the foundation for formulating pain intervention strategies for patients with chronic neuropathic pain.Methods:A total of 340 inpatients admitted to the Department of pain and spinal surgery in two Grade A hospitals in Jinan City,Shandong Province from February 2022 to August 2022 were selected by convenience sampling method.The questionnaires included the Social demographic and disease questionnaire,Pain Catastrophizing Scale(PCS),Alexithymia Scale-20(Toronto Alexithymia Scale-20,TAS-20),the 10-item Connor-Davidson Resilience Scale(CD-RISC-10),The Self-Report Symptom Inventory(Symptom Check-List90,SCL-90),and Numerical Rating Scale(NRS),which were used to measure Socio-demographic and disease conditions,pain catastrophizing,alexithymia,psychological resilience,somatization and pain degree of chronic neuropathic pain patients,respectively.SPSS25.0 and Process plug-in were used for statistical analysis.The main statistical methods included descriptive statistical analysis,independent sample t-test,one-way analysis of variance,Kruskal-Wallis H nonparametric test,Pearson correlation analysis,Hierarchical regression analysis and Bootstrap mediation effect test.Results:1.The total score of pain catastrophizing in patients with chronic neuropathic pain ranged from 0 to 52,with an average score of 30.09±15.75,and the average score of total items was 2.31±1.21.The average scores of three dimensions from high to low wererumination,helplessness and exaggeration.2.Univariate analysis showed that pain catastrophization in patients with chronic neuropathic pain was statistically different in age,family monthly income,disease type and pain score(all P<0.05).3.Correlation results showed that alexithymia(r=0.38)and somatization(r=0.27)were positively correlated with pain catastrophizing(P<0.01);Resilience was negatively correlated with pain catastrophizing(r=-0.27,P<0.01);Alexithymia is correlated with resilience(r=-0.33,P<0.01)and somatization(r=0.17,P<0.01).4.Multiple linear regression results showed that pain catastrophizing in patients with chronic neuropathic pain was affected by pain score,disease type,alexithymia,resilience and somatization(all P<0.05).5.The resilience(β=-0.15,P<0.01)and somatization(β=0.20,P<0.001)have parallel mediating roles on the relationship between alexithymia and pain catastrophizing in patients with chronic neuropathic pain.The indirect effect value was both 0.05,accounting for 15.63%of the total effect size,and the Bootstrap 95%CI was(0.02,0.09)and(0.02,0.08),respectively.Conclusions:1.Patients with chronic neuropathic pain have a higher level of pain catastrophizing,which is worthy of attention.2.The higher the age,the lower the monthly family income,the higher the pain score,the more severe the pain catastrophizing level.Pain catastrophizing is particularly severe in patients with trigeminal and postherpetic neuralgia.3.Pain score,disease type,alexithymia,resilience and somatization can significantly affect pain catastrophizing in patients with chronic neuropathic pain.4.Alexithymia could indirectly affect pain catastrophizing through resilience and somatizationin patients with chronic neuropathic pain.It is suggested that the level of pain catastrophizing can be reduced by improving psychological resilience and reducing somatization symptoms.
