| Objective:To investigate the correlation and clinical significance of Modic change(MC)in the cervical spine with blood lipids,blood glucose and the angle of K-line inclinationMethods:A retrospective analysis of 80 patients who were hospitalized for neck diseases in our hospital from January 2015 to January 2021 and met the inclusion and exclusion criteria were retrospectively analyzed.According to whether the cervical vertebral body endplates have signal changes on MRI,they were divided into groups with Modic changes(MC+)and groups without Modic changes(MC-).Experimental data collection:(1)Collect general information according to the patient’s medical history:age,gender,smoking history,and drinking history;(2)Measure the sagittal parameters of the cervical spine according to the lateral X-ray of the patient’s cervical spine:K-line inclination,C2-C7 sagittal The axial vertical distance(C2-C7 SVA),T1 inclination angle,C2-C7 Cobb angle;(3)Blood lipid parameters were collected according to the patient’s test results:high-density lipoprotein(HDL),low-density lipid Protein(low-density lipoprotein,LDL),lipoprotein a(lipoprotein(a),LP(a)),total cholesterol(TC),triglyceride(triglyceride,TG);(4)According to patient test results Collect blood glucose parameters:glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG).Statistical analysis:Chi-square test was used to compare the gender,smoking and drinking between MC(+)and MC(-)groups;independent samples t test and nonparametric test were used to compare age,blood lipids,blood glucose and sagittal parameters.Differences between MC(+)group and MC(-)group;Pearson correlation analysis of the correlation between Modic changes and various parameters;ROC curve analysis of the effects of experimental parameters with significant correlation with Modic changes on cervical Modic changes and their effects threshold.Results:(1)Differences of parameters between Modic-positive group and Modic-negative group:(1)General information:there was no significant difference in age,gender,smoking history,and drinking history between the two groups(P>0.05);(2)Blood lipids,blood glucose parameters:total cholesterol,triglyceride,glycosylated hemoglobin,fasting blood glucose were significantly different between the two groups(P<0.05),high density lipoprotein,low density lipoprotein,lipoprotein a had no significant difference between the two groups difference(P>0.05).(3)Cervical spine sagittal parameters:K-line inclination angle and C2-C7 Cobb angle were significantly different between the two groups(P<0.05),while T1 inclination angle and C2-C7 SVA were not significantly different between the two groups(P<0.05).p>0.05);(2)The correlation between Modic changes and various parameters:(1)Modic changes were significantly positive with age,total cholesterol,triglyceride,glycosylated hemoglobin,fasting blood glucose,K-line inclination angle,and T1 inclination angle(r>0,p<0.05),and was significantly negatively correlated with C2-C7 Cobb angle(r<0,p<0.05);(2)Modic changes were associated with gender,smoking history,drinking history,high-density lipoprotein,low density lipoprotein,lipoprotein a,C2-C7 SVA had no significant correlation(p>0.05).(3)ROC curve analysis of the influence of glycated hemoglobin and K-line inclination angle on Modic changes showed that the sensitivity and specificity of the glycated hemoglobin ROC curve were high,the area under the curve(AUC)was 0.84,and the diagnostic critical value was 6.5(P<0.05);The K-line tilt angle ROC curve had high sensitivity and specificity,the area under the curve(AUC)was 0.80,and the diagnostic critical value was 6.01(P<0.05).Conclusion:(1)Cholesterol,triglyceride,glycosylated hemoglobin,and fasting blood glucose are closely related to cervical Modic changes.Controlling blood lipid and blood sugar levels will reduce the risk of cervical Modic changes.(2)The inclination angle of K-line is significantly positively correlated with the Modic change of the cervical spine,and a larger K-line inclination angle may lead to the Modic change of the cervical spine.(3)The ROC curve shows that the glycated hemoglobin and K-line inclination have higher diagnostic value for cervical Modic changes.When the glycated hemoglobin is greater than 6.5 and the K-line inclination is greater than 6.01,the occurrence of cervical Modic changes should be alerted. |
