| Objective:To investigate the significance of immunohistochemical detection of Mycobacterium tuberculosis secretory antigens Ag85B,ESAT-6 and CFP10 in the diagnosis of pulmonary tuberculosis.Methods:47 cases of pathologically confirmed pulmonary tuberculosis tissue specimens in the Department of pathology,Affiliated Hospital of North Sichuan Medical College from April 2019 to October 2021 were selected as the experimental group,and 48 cases of non-tuberculosis specimens were selected as the control group.Immunohistochemical staining(SP)was used to detect the expression of Ag85B,ESAT-6 and CFP10 in the above tissues,and the significance and application value of three proteins in the diagnosis of tuberculosis were analyzed.Results:1.Immunohistochemical method was used to detect Ag85B protein,ESAT-6 protein and CFP 10 protein in pulmonary tuberculosis lesions.The location of positive signals was related to the distribution of acid-fast bacilli,and the range was wider;The positive signal intensity was related with the number of acid-fast bacilli,suggesting that this method might be applied to tuberculosis diagnosis.2.The immunohistochemical results of 47 cases of pulmonary tuberculosis and 48 cases of non-tuberculosis:Ag85B was 65.96%sensitive and 83.33%specific for diagnosing pulmonary tuberculosis;ESAT-6 was 74.47%sensitive and 89.58%specific for diagnosing pulmonary tuberculosis;The sensitivity of CFP10 for diagnosing pulmonary tuberculosis was 63.83%,and the specificity was 93.75%.3.The results of 19 pulmonary tuberculosis specimens with acid-fast staining:the sensitivity of Ag85B,ESAT-6,and CFP10 immunohistochemistry were 84.21%,73.68%,and 63.16%,respectively,which were significantly higher than the positive rate of acid-fast staining 52.63%.4.The results of combined protein detection:the sensitivity of Ag85B combined with ESAT-6 was 85.11%,the specificity was 81.25%;the sensitivity of Ag85B combined with CFP10 was 82.98%,the specificity was 81.25%;the sensitivity of ESAT-6 combined with CFP10 was 80.85%,the specificity was 85.42%;the sensitivity of the three proteins combined with immunohistochemical staining was 87.23%,and the specificity was 75.00%.Conclusion:1.Immunohistochemical detection of Ag85B,ESAT-6 and CFP1Oin lung tissuemay be used for the diagnosis of tuberculosis.2.Compared with acid-fast staining,immunohistochemical detection of Ag85B,ESAT-6 and CFP10 was more sensitive.3.The Combined test may be more meaningful in the diagnosis of tuberculosis than the three antigen test alone. |
