Research purpose: To understand the changes in healthcare manpower in primary healthcare institutions in H County from 2015 to 2020 and the current situation in 2020.Based on the results of structural data analysis,identify the problems in human resources of grassroots medical and health institutions in H County.Using the theory of health equity to evaluate the fairness of health resources enjoyed by residents in H County.Based on the theory of resource allocation,corresponding solutions are proposed to provide reference for the improvement of the service capacity of grassroots medical and health institutions in H County.Research method: Data sources were obtained through interviews and questionnaire surveys conducted on township and village clinics in H County,Luzhou City.The fairness analysis adopts the Gini coefficient method,and the existing problems are found by comparing the national average level to find the shortage of human resources in the grassroots medical and health institutions in H County.Research results: In terms of total number,the scale of health technical personnel in primary medical and health institutions in H County expanded from 2015 to 2020,with a total increase of34.26%.However,general practitioners and rural doctors still face the dilemma of insufficient numbers.Structurally,data analysis was conducted from the perspectives of professional categories,technical qualifications,educational background,age,length of service,and gender.It was found that the level of technical qualifications,educational background,and age structure have improved,but indicators such as the number of general practitioners per 10000 population and the number of technical personnel in clinics per 1000 rural population are still lower than the national average.In terms of distribution,the fairness of the distribution of health manpower in H County’s grassroots medical and health institutions was evaluated from three aspects: total distribution,distribution of practice categories,and distribution of resources serving "one old and one small".The evaluation results were good.Research conclusion: Based on the analysis of survey data,it is found that the total number of health personnel in the county has increased rapidly,the allocation is relatively reasonable,and the fairness of residents’ access to health resources is good.However,there are still problems such as a shortage of general practitioners and traditional Chinese medicine talents,severe brain drain,low educational level,ineffective improvement of professional title level,insufficient number of personnel in village level institutions,and age gaps.By sorting out the human resource policies of national and Sichuan medical and health institutions,we have found methods suitable for improving the human service capacity of grassroots medical and health institutions in H County.It is recommended that the government and grassroots medical and health institutions take measures to alleviate talent loss,increase the training of general practitioners and traditional Chinese medicine talents,enhance the education and technical qualifications of health personnel,and expand the number of personnel in village level institutions,in order to enhance the service capacity of grassroots medical and health institutions in H County.
