Objective To evaluate the value of O-RADS ultrasound classification and simplified score table in the differentiation of benign and malignant ovarian adrenal tumors.Methods A total of 189 patients with ovaria-adrenal mass indicated by transvaginal ultrasonography in our hospital were selected.The O-RADS ultrasound classification was used to classify the masses of all patients.The ROC curve was drawn with pathological results as the gold standard to analyze the value of O-RADS in differentiating benign and malignant ovaria-adrenal tumors.Then the ultrasonic characteristic factors of O-RADS in the benign group and the malignant group were compared,including the maximum diameter of the mass,multilocular mass,papillary protrusion in the inner wall,solid component of the mass,irregular thickening of the inner wall,and the factors with statistical differences were analyzed by multivariate Logistic regression.Based on the results of multi-factor regression analysis,the simplified O-RADS analysis process was proposed,and the value of simplified scoring table and O-RADS classification in the diagnosis of benign and malignant ovaria-adrenal tumors was compared.Results 1.The AUC of O-RADS for differential diagnosis of benign and malignant ovaria-adrenal tumors was 0.905,and the best cut-off value was ≥5 types of O-RADS.In this case,the diagnostic sensitivity,specificity,positive predictive value and negative predictive value of this method were 72.09%,93.84%,71.5%and 91.9%,respectively.2.Compared with the benign group and the malignant group,there were statistical differences in the ultrasonic characteristic parameters of O-RADS,such as the maximum diameter of the mass,multilocular mass,papillary protrusion on the inner wall,solid component of the mass,blood flow score and irregular thickening of the inner wall(all P<0.05).3.Multivariate Logistic regression analysis showed that blood flow score and solid mass component were independent risk factors for predicting ovaria-adrenal malignancy(all P<0.05).4.The AUC of differential diagnosis of benign and malignant ovaria-adrenal tumors by O-RADS simplified scoring table was 0.985,and the optimal cut-off value was ≥7 points.When the total score was ≥7 points,The sensitivity,specificity,positive predictive value,negative predictive value were 88.37%,97.26%,90.5%and 96.6%,respectively.The diagnostic efficacy of O-RADS simplified scale was significantly higher than that of O-RADS classification(0.985 vs.0.905),and the difference was statistically significant(Z=4.190,P<0.001).Conclusion 1.O-RADS classification is of high value in the differential diagnosis of benign and malignant ovaria-adrenal tumors.2.In O-RADS classification,solid component and mass blood flow score were independent risk factors for ovarian adrenal malignancy.3.The simplified O-RADS grading table is of higher value for the diagnosis of benign and malignant ovary-adrenal tumors,and it is simpler than the original O-RADS classification,which may provide a new idea for clinical promotion of O-RADS classification.
