| BackgroundAlcohol dependence(AD)has become a common mental illness in today’s society,with patients often in a cycle of withdrawal and relapse.In this process,’craving’ plays a crucial role,and if the patient has no craving for alcohol,they will naturally not drink again.At present,in clinical practice,the measurement of craving for alcohol addicts is mainly through scales,and patients are prone to hiding their true mentality under subjective conditions.Event related potentials(ERP)are an objective tool that reflects cognitive processing through task state EEG activity and may be related to alcohol craving.Therefore,alcohol clue response induced ERP can be used to study alcohol craving.Objective1.To study ERP characteristics related to alcohol cue response induced craving in AD patients.2.The correlation between ERP components and drinking characteristics and subjective craving degree of AD patients was analyzed.3.To explore the specific electrode site desired by ERP monitoring AD patients.MethodsThirty AD patients hospitalized in the Addiction Department of Shenzhen Kangning Hospital from December 2021 to December 2022 were included in this study.Each patient watched two groups of visual cues containing alcohol and scenery to induce ERP.The Visual Analogue Scale(VAS),Alcohol Urgency Questionnaire(AUQ),and steps of approaching alcohol adapted from overcoming arachnophobia were used as behavioral craving tests to evaluate subjective craving.The EEG data was preprocessed by standardization,and the total average of ERP of the subjects under two kinds of stimulation conditions was carried out respectively in the group,and the EEG and EEG topographic maps were derived.The difference analysis of ERP data between the two groups was performed by two-factor analysis of variance.Pearson and Spearman correlation analysis were performed between ERP data and drinking characteristics and scale scores in the alcohol-dependent group.Chi-square test and T test were performed between follow-up repeated drinking and non-repeated drinking groups and scale scores of drinking characteristics.Results1.All the subjects were healthy middle-aged males with normal cognition,mild depression and low craving score.There was a significant positive correlation between the SAS scale and the PACS scale(P<0.001),a significant positive correlation between the SDS scale and the PACS scale(P<0.05),a significant positive correlation between the SDS scale and the SAS scale(P<0.001),a significant positive correlation between the MoCA scale and SAS and SDS scales(P<0.001),and a significant positive correlation between the AUQ scale and SAS and SDS scales(P<0.05),There is a significant negative correlation between alcohol addiction history and the AUQ scale(P<0.001)2.The induced ERP components were mainly negative phase potential N200(150-250ms)and positive phase potential P300(250-600ms).3.There were significant differences in ERP induced by the two stimuli.The N200 amplitude was smaller and the latency was significantly shorter in frontal Fz(P=0.003)and central Cz(P=0.006)under the stimulation of alcohol images.The amplitude of P300 was larger and significant in frontal Fz and parietal Pz(P=0.03;P=0.006),P300 incubation period was shorter and significant in frontal Fz and parietal Pz(P=0.022;P=0.041).4.The difference in activation of N200 was mainly in the central region of Cz,while the difference in activation of P300 was mainly in the top region of Pz.alpha waves of 8-13Hz appeared in the time-frequency maps of Fz,Cz and Pz,which were correlated with the cognitive time Windows of N200 and P3005.The P300 amplitude of Pz in the alcohol cue was positively correlated with Psqi score(r=0.880,P<0.001),and the P300 amplitude of Pz in the N200/top zone of Fz in the scenic cue was negatively correlated with Psqi score(r=-0.850,P<0.05;r=-0.806,P<0.05),and the N200 latency of Fz in frontal area was positively correlated with MoCA score(r=-0.990,P<0.001).6.The follow-up comparison between the redrinking and abstinence groups showed that marital status(P=0.037),years of education(P=0.027)and VAS score(P=0.006)may affect the redrinking rate of AD patients.Conclusions1.Traditional alcohol craving scales and methods have a large error in the measurement of cravings in AD patients;2.Visual cues containing alcohol cues can induce craving related ERP,and monitoring this task-state EEG response can make up for the shortcomings of subjective assessment methods,and can be used as an objective method to assess alcohol cravings in AD patients.3.ERP monitoring the craving of AD patients can optimize the collection of EEG,centralized collection of Fz in the forehead,Cz in the central area and Pz in the parietal area,supplemented by the collection of electrodes around Fz,Cz and Pz,which greatly reduces the daily workload compared with the usual 32/64 hole collection. |
