Dose-response Associations Of Physical Activity And Sedentary Behavior Patterns With Chronic Pain In Community-Dwelling Older Women

Objective:Objective measurement methods are used to explore the dose-response relationship between Sedentary Behavior(SEB)and Physical Activity(PA)and their patterns on chronic pain,number of pain sites and pain degree in elderly women in communities.Explore 30 minutes of SEB,low intensity Physical Activity(LPA)and Moderate to Vigorous Physical Activity(LPA)using isochronous substitution model The correlation between MVP A)substitution and chronic pain was determined to determine whether SEB and PA were the optimal cut-off values for determining chronic pain in the elderly,which laid a theoretical foundation for improving chronic pain in elderly women.Method:1)A cross-sectional study was conducted on 1370 community elderly women randomly selected from Yantai City,Shandong Province.The demographic,cognitive,nutritional and chronic pain data of elderly women in the community were collected face-to-face using paper questionnaires.The instrument measured Body Mass Index(BMI)and bone density among elderly women in the community.2)Use the triaxial accelerometer to collect SEB and PA data of elderly women in the community.It is required to wear the accelerometer for 7 consecutive days,and collect the accelerometer data for≥4 days and ≥10 hours a day.SEB patterns were classified into 30min-Bouted SEB(≥30 min SEB)and 60min-Bouted SEB(≥60 min SEB).PA was classified into LPA and MVP A,and PA patterns were classified into Sporadic PA(single time<10 minutes PA)and Bouted PA(single time≥10 minutes PA).3)Non-parametric test was used to explore the differences of multiple indicators between the non-chronic pain group and the chronic pain group;Multivariate Logistic regression analysis was used to explore the relationship between SEB,PA and their patterns and chronic pain,the number of pain sites and pain degree.The isochronous substitution model was used to analyze the relationship between SEB,LPA and MVPA and chronic pain.The optimal cut-off value was determined by Receiver Operating Characteristic Curve(ROC)analysis.Result:1)Of the 1095 elderly women in the community included in the analysis,778 had chronic pain.Non-parametric test showed that only BMI,Sporadic LPA,total MVP A,Sporadic MVPA,and Bouted MVPA were significantly different between chronic pain group and non-chronic pain group(P<0.05),and no differences were found in other indexes(P>0.05).2)After adjusting for multiple confounding factors,Logistic regression for different SEB and PA modes showed that the total MVPA time and Bouted MVPA in Model 1 were significantly positively correlated with chronic pain,lumbar joint and knee joint,number of pain sites and pain degree(P<0.05).In Model 2,only Bouted MVPA was positively correlated with chronic pain,the number of pain sites and pain degree(P<0.05).Both total SEB time and different SEB patterns were not associated with chronic pain.3)In the isochronous substitution model,when 30 minutes of MVPA was substituted for 30 minutes of SEB or LPA,there was a significant positive correlation with chronic pain in elderly women in the community(P<0.05);When 30 minutes of SEB was substituted for 30 minutes of LPA or MVPA,the results were not correlated(P>0.05).When 30 minutes of LPA was substituted for 30 minutes of SEB or MVPA,the results were not correlated(P>0.05).4)ROC curve analysis results showed that the optimal cut-off values of MVPA and Bouted MVPA for predicting chronic pain in elderly women in the community were 32.7 min/day and 9.57 min/day,respectively(P<0.05).Conclusion:1)There was a significant dose-response relationship between MVPA and chronic pain of knee joint and lumbar joint in elderly women in the community.A single MVPA lasting more than 10 minutes was most effective in reducing the OR value of chronic pain,the number of pain sites and pain degree in elderly women in the community.2)Replacing 30 minutes of SEB and 30 minutes of LPA with 30 minutes of MVPA can significantly reduce the level of chronic pain in elderly women in the community.3)Increasing MVPA and MVPA duration of more than 10 minutes may be an effective strategy to prevent chronic pain.It is recommended to maintain MVPA duration of more than 32.7 minutes/day,and maintain MVPA duration of more than 10 minutes/day at more than 9.57 minutes/day,which may have a positive effect on the prevention and relief of chronic pain.
