ObjectiveTo investigate the epidemiological analysis of impacted teeth in patients with malocclusion during replacement period.Statistics of three-dimensional impacted location and root development were performed on the CBCT data.Through the analysis of cephalometric measurement by the images of patients with impacted maxillary incisors,the relationship between maxillary impacted maxillary growth and development and impacted maxillary incisors was discussed,and the association of maxillary incisors impaction and sagittal skeletal features was analyzed.The purpose is to provide reference for clinical diagnosis and treatment of impacted teeth.MethodsThe study reviewed the data of 2409 children with malocclusion in the Department of Pediatric Stomatology,Shandong University Stomatological Hospital from January 2019 to October 2022.Strictly according to the inclusion and exclusion criteria,91 patients with impacted teeth were selected for statistics,an epidemiological investigation was conducted on the prevalence rate,age,sex,number and position of impacted teeth.The CBCT data of 128 impacted teeth were imported into Mimics 21.0 software in DICOM format for three-dimensional reconstruction,to statistically analyze three-dimensional position,root dilaceration,and the characteristics of the length of crown and root.The imaging data of 67 patients with maxillary impacted incisors were selected and the lateral cephalograms were fixed and measured to explore the effect of impacted maxillary incisors on maxillary growth and development,and analyze the correlation of maxillary incisors impaction and skeletal pattern in the sagittal planes.SPSS 26.0 software was used to deal with the results statistically.Results1.The prevalence rate of impacted teeth in patients with malocclusion in pediatric dental clinic was 3.78%.It occurred frequently in women,but the difference was not statistically significant.2.The prevalence rate of impacted teeth in maxilla was significantly higher than that in mandible(p<0.05),and the prevalence rate of maxillary central incisor was the highest,followed by maxillary lateral incisor and maxillary canine.The number of impacted teeth was mostly single,accounting for 72.53%.Most of the patients were 7~9 years old,accounting for 65.94%.3.In the direction of impaction,labial impaction was common;in the coronal direction,mesial impaction was more common;in the vertical direction,median impaction was more common;in the sagittal direction,inverted impaction was more common.4.The inverted group exhibited a greater prevalence of root dilaceration than the non-inverted group,and the difference was statistically significant(p<0.05).In the patients with impacted unilateral maxillary central incisors,the average length of root was lower in the impacted group than that of the contralateral homonymous group,and the average crown-root ratio of the impacted group was higher than that of the contralateral homonymous group,and the difference was also statistically significant(p<0.05).In the patients with impacted unilateral maxillary lateral incisors,the average length of root in the impacted group was significantly shorter than that in the contralateral group(p<0.05).5.Among the 67 patients with impacted maxillary incisors,ANB,ANS-Ptm and S-Ptm of one central incisor(A group),one lateral incisor(C group)and ipsilateral central and lateral incisor group(D group)were lower than normal(p<0.05).The ANS-Ptm and S-Ptm of one A group were less than C group,the SNA,NP-FH and S-Ptm of D group were less than C group,and the S-Go was more than C group(p<0.05).6.Among the 67 patients with impacted maxillary incisors,the skeletal class III was the most,accounting for 47.76%.Compared with the ratios of sagittal skeletal features of Chinese stomatological in 2000 from the 7 areas teenagers,there was significant difference(p<0.05).The second was skeletal class I,accounting for 32.84%and the skeletal class II was the least,accounting for 19.40%.Conclusion1.The prevalence of impacted teeth in pediatric dental clinic was 3.78%,which mainly occurred in the area of upper anterior teeth.The prevalence rate of impacted maxillary central incisor was the highest.The upper jaw was more frequent than the lower jaw.However,no statistically relevant disparity between the sexes was observed.2.The median,mesial and the labial sides were the most frequent three-dimensional sites of impacted teeth.3.Inverted impacted teeth were more easily to be accompanied with root dilaceration.4.The root length of impacted teeth was shorter than that of normal erupted contralateral homonymous teeth,and the crown-root ratio was larger.5.The impacted maxillary incisor was related to the underdevelopment of the maxilla and the limitation of the basal bone,and the trend of skeletal class III is more obvious.The patients with one impacted maxillary central incisor tend to show skeletal class III more than the patients with only one impacted maxillary lateral incisor.
