Objective:To study the clinical effect of hemocoagulase needle occlusion in preventing complications of lung nodule biopsy,and to explore the influencing factors and preventive measures of complications of lung nodule biopsy.Materials and methods:A total of 155 patients with pulmonary nodules who underwent needle biopsy were enrolled and divided into control group(77 cases)and observation group(78 cases).After the completion of material extraction,the control group directly withdrew the needle,while the observation group,hemocoagulase-normal saline mixture was used for preventive occlusion near the lesion and needle tract when the needle was withdrawn.CT was reexamined immediately after the operation to accurately measure the proportion of pneumothorax and to grade the bleeding.The bleeding grade and the occurrence of pneumothorax between the two groups were tested by chi-square test.The proportion of pneumothorax was tested by Mann-Whitney U test.Spearman correlation analysis was used to analyze the correlation between pneumothorax,bleeding and various parameters.Results:The success rate of sampling was 100%,and the pathological diagnosis rate was 96.77%.There were no significant differences in general data,pulmonary nodules and puncture related parameters between the two groups(P>0.05).Immediate postoperative CT showed that there were 39 cases of low grade bleeding and 38 cases of high grade bleeding in the control group,and 58 cases of low grade bleeding and 20 cases of high grade bleeding in the observation group.The difference between the two groups was statistically significant(χ2 value was 9.302,P<0.05).There were 31 cases of pneumothorax in the control group and 18 cases of pneumothorax in the observation group,and the difference was statistically significant(χ2 value was 5.292,P<0.05).The difference in the proportion of pneumothorax between the two groups was statistically significant(P<0.05).The grade of pulmonary hemorrhage was negatively correlated with hemocoagulase needle tract occlusion and the percentage of solid components of pulmonary nodules,and positively correlated with the distance of needle tract(P<0.05).The occurrence of pneumothorax was negatively correlated with hemocoagulase needle tract occlusion,and positively correlated with The Times of passing through pleurae,passing through emphysema and the location of the lesion.The proportion of pneumothorax was positively correlated with The Times of passing through pleura,passing through emphysema,and the Angle between needle path and pleura(P<0.05).Conclusion:Needle occlusion with hemocoagulase can safely and effectively reduce the risk of pneumothorax and bleeding in biopsy of pulmonary nodules,which is helpful to improve the safety of biopsy of pulmonary nodules,and has good clinical application value.Hemorrhage was correlated with hemocoagulase needle tract occlusion,needle tract distance,and solid component percentage of pulmonary nodules.Pneumothorax was correlated with hemocoagulase needle tract occlusion,times of passing through pleura,passing through emphysema,lesion location,needle tract Angle and pleura.
