| Objective:This article aims to collect and consult a large number of ancient and modern literature,organize and study ancient medical books related to aphonia,systematically sort out the disease names,causes,pathogenesis,symptoms,and other contents of aphonia in various historical periods,and summarize and analyze the treatment principles and methods of aphonia,in order to comprehensively grasp the principles,methods,formulas,and medicines of aphonia syndrome treatment,provide certain reference for theoretical research and clinical treatment of aphonia,and comprehensively improve the study of ancient medical books on aphonia.Methods:This paper uses the method of literature research,takes the electronic series of Chinese Medical Code,the ancient book database of Chinese Medical Code and the collection of books in the library of Nanjing University of Chinese Medicine as data sources,and combines the method of historical analysis to collect and sort out the literature related to aphasia in previous dynasties;Refer to journal literature and papers related to this paper;Refer to other relevant non medical literature to enrich and enrich the research of this topic.According to the collected literature,the development of Traditional Chinese medicine’s understanding of aphasia is preliminarily outlined according to the disease name,etiology and pathogenesis,symptoms and manifestations of aphasia.Secondly,the discussion on the differentiation and treatment of aphasia in ancient medical books is summarized,and the different angles of differentiation and treatment of aphasia and corresponding prescriptions are learned,so as to have a comprehensive understanding of the content of the differentiation and treatment of aphasia in ancient medical books.Results:1.Firstly,the definition of aphonia was determined,and the scope of research on aphonia in this article was defined.2.This study systematically reviews the discussions on aphonia in ancient medical literature from the Pre Qin to the late Qing Dynasty,and summarizes the understanding of ancient physicians on pronunciation mechanism,name of aphonia,etiology,pathogenesis,and clinical manifestations.(1)Pronunciation mechanism:the pioneer of the theory of phonetics in the Neijing,and later to the Song,Jin,and Yuan dynasties,Li Dongyuan,Liu Wansu Yang Shiying proposed the viewpoints of "combining the heart and lungs,using the three jiao","forming qi and drumming",and "the main sound of the heart,lungs,and kidneys",which had a profound impact on later generations.(2)Disease name:Firstly,we selected such aphonia-related characters as "yīn"(瘖),"yǎ"(哑),"sī"(嘶)and "shà"(嗄),and clarified the similarities and differences between these similar names through the examination and interpretation of their meanings.At the same time,the origin and development of the name of aphonia were sorted out,and it was found that during the pre Qin and Han dynasties,the name of aphonia first appeared and was often named based on the characteristics of the symptoms;During the Jin and Tang dynasties,naming began to be combined with etiology,pathogenesis,and syndromes;During the Song,Jin,and Yuan dynasties,the naming of lost sounds inherited and developed the methods of previous dynasties,tending to be enriched;During the Ming and Qing dynasties,aphonia was widely involved and many special disease names emerged.(3)Etiology and pathogenesis:Complex and diverse,including the invasion of the lung system by the six exogenous factors,seasonal accumulation of qi,or obstruction of phlegm and blood stasis,obstruction of the orifices,visceral deficiency and cold,prolonged illness and fatigue,etc.The main site of onset is in the heart,lungs,and kidneys.During the Ming and Qing dynasties,doctors further improved and enriched their understanding of aphonia,established specialized articles to summarize various causes and mechanisms of aphonia.(4)Symptoms:During the pre Qin to Sui and Tang dynasties,acute aphonia was common,with throat symptoms such as "throat obstruction" and "swollen throat" being the main symptoms.During the Song,Jin,and Yuan dynasties,doctors began to attach importance to chronic aphonia.During this period,most of the throat symptoms were throat swelling and pain,and there were also many abnormal tongue movements.During the Ming and Qing dynasties,epidemics were prevalent,resulting in a high incidence of pharyngeal diseases,most of which were critical symptoms.3.This article comprehensively explores the treatment of aphonia and finds that there are rich discussions on the differentiation and treatment of aphonia in ancient medical books.It analyzes and organizes its internal and external treatment methods.In the study of the differentiation and treatment of aphasia,it is divided into "laryngeal diagnosis" and "lingual diagnosis" for Vibhanga respectively.Based on the etiology and pathogenesis,throat diagnosis can be divided into 11 syndrome types according to syndrome types:wind cold attacking the surface syndrome,wind fire invading the lung syndrome,fire heat stagnation syndrome,cold evil enveloping fire syndrome,phlegm turbidity internal obstruction syndrome,qi stagnation and blood stasis syndrome,lung yin depletion syndrome,kidney deficiency and fire excess syndrome,lung kidney deficiency and kidney deficiency syndrome,lung qi deficiency and weakness syndrome,and spleen qi weakness syndrome;The tongue diagnosis is divided into 4 syndrome types:superficial deficiency and pathogenic middle syndrome,wind phlegm obstruction of the orifices syndrome,blood stasis obstruction of the orifices syndrome,and kidney essence deficiency syndrome,and discussed separately.In the external treatment,there are different external treatment methods such as soaking,blowing medicine,exploring vomiting,sticking,and acupuncture and moxibustion treatment.4.This article discusses the relationship between aphonia and the five organs,ancient related diseases of aphonia,and modern related diseases of aphonia.Among the relationships between the five organs and aphonia,tongue diagnosis is mainly related to the four organs of the heart,liver,spleen,and kidney;Laryngeal diagnosis focuses on the lungs and is also related to multiple organs.The ancient related diseases of aphonia include some lung diseases,laryngeal diseases,stroke,as well as other diseases such as fatigue,edema,and syphilis.The modern related diseases of aphasia include vocal nodules,vocal polyps,Vocal cord cyst,acute and chronic pharyngitis,gastroesophageal reflux disease,hypothyroidism,etc.Conclusion:This article systematically organizes and studies ancient medical records related to aphonia,sorts out the disease names,causes,pathogenesis,and symptoms of aphonia in different historical periods,analyzes and summarizes the differential treatment and external treatment methods of different types of aphonia,and discusses the related issues of aphonia. |
