Management Status And Optimization Plan Of High-risk Pregnancy And Pregnant Women In A Tertiary General Hospital In Foshan City

Research Objectives:To understand the policy environment of management of pregnant women and the current situation of management of pregnant women in the case hospital,identify the risk links in the management process of high-risk pregnant women,analyze the reasons and propose targeted intervention measures,and determine the optimization plan of management of high-risk pregnant women in the case hospital.To explore the application effect of research methods and tools in the optimization research of high-risk pregnancy management.Research methods:1.Investigation on high-risk pregnancy of pregnant women in case hospitals:General information of hospitalized obstetric patients,high-risk pregnancy cases,elderly pregnancy cases,floating population pregnant women cases,high-risk factors and sequence data were collected through the hospital information system by mathematical statistics method to analyze the occurrence of high-risk pregnancy.2.Investigation on the management status of high-risk pregnancy and pregnant women in the case hospital:(1)Sorting out the document system of the case hospital to clarify the management structure and process of high-risk pregnancy and pregnant women;(2)Adopting the semi-structured interview method to understand the existing problems in management and the needs of medical staff for the improvement of the current situation.3.Risk identification of high-risk pregnancy and pregnant women management in the case hospital:Based on HFMEA,the high risk pregnancy and pregnant women management process execution risk priority index score,determine the high-risk failure mode and failure reasons.4.Construction of optimal management plan for high-risk pregnancy and pregnant women in case hospitals:Intervention measures were proposed through literature policy research and brainstorming.The importance and feasibility of intervention measures were scored,modified and supplemented by the expert letter consultation method,and the optimal management plan of high-risk pregnancy and pregnant women in the case hospital was finally determined.Research results:1.A total of 8,888 obstetric inpatients were included in the survey of high-risk pregnancy in the case hospital from June 2018 to May 2022.There were statistical differences in the occurrence of high-risk pregnancy,the distribution of old age and the distribution of floating population,and the sequence of high-risk factors changed for four consecutive years.2.The management process of high-risk pregnancy and pregnant women in the case hospital includes 5 main processes and 21 sub-processes.7 high-risk failure modes and 13 failure causes were identified and screened.3.In view of high-risk failure modes and causes,the HFMEA team formulated 37 intervention measures;Through two rounds of expert evaluation and supplement,27 intervention measures were finally determined,forming an optimization plan for high-risk pregnancy and pregnant women management in the case hospital.Research conclusion:1.The incidence rate of high-risk pregnancy in the case hospital is increasing year by year,and the risk of pregnancy is escalating,so the hospital faces challenges in ensuring the safety of pregnant women.2.The management of high-risk pregnancy and pregnant women in the case hospital has problems such as shortage of human resources in obstetrics,poor operation of multidisciplinary cooperation mechanism,inadequate implementation of maternal safety management responsibilities,which may bring threats to maternal and child safety and hospital management risks,and there is still a gap between medical institutions to provide safe,effective,convenient and warm high-quality maternal and child health services.3.The application of risk management,real world research,HFMEA and other theories and tools helps to identify the risk of high-risk pregnancy and maternal management process,formulate intervention measures,and provide a set of scientific and standardized optimization research model and reasonable and feasible optimization scheme.
