| Objective: This clinical study aims to apply the combination of acupuncture and Xuanlung Tongqi method to treat AECOPD with constipation(phlegm-heat and internal organs)through a controlled design of clinical cases,to observe and statistically analyze the indexes of patients before and after treatment, and to evaluate the clinical efficacy and safety of the combination of acupuncture and Xuanlung Tongqi method in the treatment of AECOPD with constipation,in order to provide strong clinical evidence for the combination of acupuncture and medicine and Chinese and Western medicine in the treatment of AECOPD with constipation. The clinical efficacy and safety of the treatment of AECOPD with constipation were evaluated. Methods: According to the criteria of nadir,60 patients with AECOPD with constipation(phlegm-heat and internal organs)who were hospitalized in the Department of Pulmonary Diseases of Shandong Provincial Hospital were selected and randomly divided into a test group and a control group using the random number table method, with 30 cases in each group. In the control group, lactulose was added, and in the experimental group, snap acupuncture(selected points: Lung Yu,Large Intestine Yu,Quchi,Fenglong,Tianshu,Zhigou,Foot Sanli,Shangjiu)was used in combination with Xuanbai Chengqi Tang(Xuanbai Chengqi Tang)and Qingjin Huayu Tang(Qingjin Huayu Tang plus reduction). The Chinese medicine symptoms scores, constipation quality of life scale scores,CAT scores and m MRC scores, and blood gas analysis indexes were recorded before and after treatment in both groups, and the data were processed and analyzed using the statistical software SPSS 26.0.Results:This study finally completed the observation of 60 patients(30 patients in the test group and 30 patients in the control group)with AECOPD with constipation(phlegm-heat and internal organs evidence),and there were no rejections and shedding cases in both groups.1.Baseline comparison: Analysis of general information and pre-treatment indexes of patients in the two groups showed no statistically significant differences(P>0.05).2.Comprehensive clinical efficacy and evidence points of Chinese medicine: the effective rate of the test group(93.33%)was better than that of the control group(86.67%),and the difference was statistically significant(P<0.05).Both groups were able to reduce the individual TCM symptom scores and total scores(P<0.01),and the test group was more advantageous than the control group in improving the scores of cough, sputum, wheezing, fever, dry mouth and rales(P<0.05),and there was no statistically significant difference between the two groups in improving abdominal distension and dry stools(P>0.05).3.constipation quality of life scale scores: after treatment, the constipation scores of both groups were significantly lower than those before treatment(P<0.01),and the difference between the two groups was not statistically significant(P>0.05).4.CAT score and m MRC score: after treatment, the scores of both groups were significantly lower than those before treatment(P<0.01),and the improvement of the test group was better than that of the control group, and the difference was statistically significant(P<0.05).5.Blood gas analysis index: After treatment,Pa O2 and Pa CO2 values in both groups improved significantly compared with those before treatment(P<0.01),and Pa O2 values increased and Pa CO2 values decreased compared with those before treatment, and the improvement of Pa O2 and Pa CO2 levels in the test group was significantly better than that in the control group(P<0.05).6.Safety evaluation: No significant adverse reactions were observed during the treatment with snap acupuncture combined with the method of promoting lung and internal organs, and the application was safe and reliable. Conclusion:From the analysis of the trial data,it was concluded that on the basis of the conventional western medical treatment of AECOPD,the addition of acupuncture combined with the Xuanlung Tongqi method in the trial group significantly improved the overall efficiency of the treatment of AECOPD with constipation(phlegm-heat and internal organs)compared with the addition of lactulose in the control group, significantly relieved the clinical symptoms such as cough, sputum, asthma, dry stool and abdominal distension, corrected hypoxia and carbon dioxide retention in the body, improved the m MRC and CAT scores, and improved the m MRC and CAT scores.m MRC and CAT scores, and improved exercise tolerance and quality of life of patients. The combination of acupuncture and lung circulation method was effective and efficient in treating constipation in patients with AECOPD(phlegm-heat and internal organs),and the efficacy was comparable to that of oral lactulose. |
