ObjectiveObjective To investigate the correlation between the ratio of neutrophil to lymphocyte(NLR),systemic immunoinflammatory index(SII,platelet count ×neutrophil count/lymphocyte count)and red blood cell distribution width(RDW)before intravenous thrombolytic therapy of patients with acute cerebral infarction and the prognosis of intravenous thrombolytic therapy in patients with acute cerebral infarction within 6 hours of onset.Methods1.A total of 100 patients with acute cerebral infarction were 2021 treated with intravenous urokinase thrombolysis within 6 hours of onset in Jianping County Hospital of traditional Chinese medicine from January to September2022.2.The relationship between the prognosis and laboratory parameters,such as white blood cell count(WBC),neutrophil count(NEUT),lymphocyte count(Lym),platelet count,red blood cell distribution width(RDW),blood Low-density lipoprotein,blood glucose and NIHSS score were studied retrospectively.3.Three months after the onset of the disease,the patients were followed up and divided into good prognosis group and bad prognosis group according to the modified Rankin Scale.4.SPSS 26.0 software was used for statistical analysis.If the measured data were in accordance with the normal distribution,the X ± s expression was used.Two independent samples t test was used for comparison between the two groups.Frequency(percentage)was used to count data,and chi-square test was used to compare groups.Multivariate analysis was performed by using binary logistic regression analysis.Sperman correlation analysis was used for correlation analysis.P & LT;0.05 was statistically significant.Results1.In patients with intravenous thrombolysis in acute cerebral infarction,patients with a history of smoking before onset were more likely to have a poor prognosis,and the difference was statistically significant compared with those without a history of smoking(P & Lt;0.05).2.The time from onset to thrombolysis and NIHSS score in the poor prognosis group were higher than those in the good prognosis group(P & LT;0.05),but there were no significant differences in age,systolic and diastolic blood pressure(p & GT;0.05).3.The neutrophil count,SII value,NLR value,and RDW value in patients with poor prognosis were higher than those in patients with good prognosis,and the difference was statistically significant(P<0.05).Through binary regression,it was found that the above indicators are independent risk factors for prognosis judgment.4.The SII,NLR,RDW and smoking history of patients with acute cerebral infarction within 6 hours after onset were predicted by the ROC curve of risk prediction model,the specificity and sensitivity of the combined prediction were higher.Conclusion1.NLR,SII and RDW before thrombolysis are closely related to the prognosis of patients with ACI.They are independent risk factors for the outcome of intravenous thrombolysis in ACI,which can be used as a predictor of the outcome.We also found that NLR,SII and RDW can predict the outcome by the ROC curve of each risk factor,more efficient.2.The prognosis of ACI patients treated with intravenous thrombolytic therapy is also related to other indexes,such as smoking history,the time from onset to intravenous thrombolytic therapy,and NIHSS score before intravenous thrombolytic therapy,it is suggested that NLR,SII,RDW value combined with these indexes can better predict the prognosis.3.There was no correlation between SII,NLR and TOAST classification,so there was no difference in etiology.
