Clinical Observation On The Treatment Of Generalized Anxiety Disorder And Clinical Effect Of Shiwei Wendan Decoction From The Deficiency Of Heart Kidney Essence And Phlegm Turbidity-Gallbladder Qi

Objective: This study aims to explore the mechanism of generalized anxiety disorder from the perspective of "heart-gallbladder qi deficiency,kidney essence deficiency,phlegm turbidity and internal disturbance",observe and evaluate the clinical efficacy and safety of Shiwei Wendan Decoction in the treatment of generalized anxiety disorder,in order to provide new clinical evidence and differential treatment methods for the diagnosis and treatment of generalized anxiety disorder in modern Chinese medicine.Contents and methods: A randomized controlled trial was conducted to include 66 patients with generalized anxiety disorder who were admitted to the encephalopathy Department of Shandong Hospital of Traditional Chinese Medicine from October 2021 to November 2022 and were randomly divided into two groups: the treatment group was given Shiwei Wendan decoction and the control group was given paroxetine hydrochloride.The treatment cycle was4 weeks.The changes of HAMA total score,SAS total score and TCM syndrome score before and after the two groups were observed,and the occurrence of adverse events during treatment was monitored.SPSS26.0 software was used for statistical analysis,and the clinical efficacy and safety of the two groups of treatment methods were evaluated.Results: 3 cases fell off during the treatment,and a total of 63 patients were included in the statistics,including 31 in the treatment group and 32 in the control group.1.Total effective rate of HAMA score:The total effective rate of the treatment group was 77.42% and that of the control group was 56.25%.Inter-group comparison of the HAMA and SAS scores between the two groups showed statistically significant differences,and the improvement of the treatment group was better than that of the control group.2.Curative effect of TCM syndromes: the total effective rate of TCM syndromes in the treatment group was 83.87%,and that in the control group was 56.25%.The comparison between the total groups of TCM syndromes after treatment was P < 0.01,and the difference was statistically significant.In addition,In the treatment group,the symptoms of good shock and easy fear(P = 0.02 < 0.05),sensitive tension(p < 0.01),dizziness and vomiting(P = 0.01 < 0.05),palpitation and shortness of breath(p < 0.01),fatigue(P = 0.02 < 0.05),waist and knee tenderness(p< 0.01),loss of appetite(p = 0.01 < 0.05)Both were better than the control group.Conclusions: Shiwei Wendan Decoction can effectively reduce the HAMA and SAS scores of patients with generalized anxiety disorder heartgallbladder qi deficiency,kidney essence deficiency,phlegm turbidity and internal disturbance syndrome,significantly improve the TCM syndrome of patients,and no obvious adverse reactions were observed during treatment,which is worthy of further research and application.
