Based On The Theory Of "Gut Retinal Axis" Observe The Clinical Effect Of Huacaishenlingbaizhu Powder Combined With Anti-VEGF In The Treatment Of WAMD

ObjectiveBy comparing and observing the clinical efficacy of Huacaishenlingba-izhu powder combined with anti-VEGF preparation and single injection of anti-VEGF preparation in the treatment of w AMD,the clinical effect of combining Chinese and Western medicine based on the theory of“Gut-Ret-ina Axis”in the treatment of w AMD is evaluated,providing new treatment ideas and methods for the disease.MethodsIn this study,60 cases(60 eyes)of w AMD diagnosed with spleen defic-iency and dampness were included and divided into control group and trea-tment group by random number method,with 30 cases(30 eyes)in each gro-up.All cases were monocular.If both eyes were affected at the same time,they were randomly selected and included.The control group was treated with the"3+PRN"regimen,that is,0.5mg Compaacept ophthalmic injection was injected into the vitreous cavity once a month after the study enter-ed the experiment,for a total of 3 months,and the efficacy was evaluat-ed after 3 months of treatment.The treatment group was additionally trea-ted with Huacaishenlingbaizhu powder for 3 months on the basis of control group.Observe and record two groups of patients before treatment,after January,February and march the best corrected visual acuity、Central ma-cular retina thickness、fundus retinal thickness seepage condition and T-CM syndrome integral.Results1.BCVAThe BCVA(Log MAR)values of the treatment group and the control group before treatment were 0.48±0.13 and 0.57±0.20,respectively,with no statistical significance(P=0.41>0.05).After treatment,BCVA(Log-MAR)values of the treatment group and the control group were 0.10±0.08and0.22±0.06,respectively,and the difference was statistically significa-nt(P=0.000<0.05).Compared between the two groups,the curative effect of the treatment group was better than that of the control group,and the difference was statistically significant(P=0.000<0.05).2.CMTThe CMT values of the treatment group and the control group before treatment were 440.87±61.184um and 438.83±54.175um,respectively,with no statistical significance(P=0.109>0.05).After treatment,the CMT val-ues of the treatment group and the control group were 203.63±30.483um and235.80±41.094um,respectively.Compar ed between the two groups,the curative effect of the treatment group was better than that of the contr-ol group,and the difference was statistically significant(P=0.000<0.05).3.Fundus exudationThe area of fundus exudation before treatment was 2.27±0.583mm~2 and2.07±0.740mm~2 in the treatment group and the control group,respectively,with no statistical significance(P=0.294>0.05).After treatment,the ar-ea of fundus exudation in the treatment group and the control group was0.43±0.774mm~2 and 0.97±0.964 mm~2,respectively.Compared between the two groups,the curative effect of the treatment group was better than that of the control group,the difference was statistically significant(P=0.001<0.05).4.The total score of TCM syndromeThe total scores of TCM syndroms in the treatment group and the cont-rol group before treatment were 11.47±2.751 and 11.60±1.812,respectiv-ely;After 3 months of treatment,the total score of TCM syndrome decreas-ed significantly,which was 3.27±1.721 and 8.97±1.159 in the treatment group and the control group,respectively.Symptom score reduction rate was71.49%in the treatment group and 22.67%in the control group.Compar-ed between the two groups,the effect of the treatment group was better than the control group,and the difference was statistically significant(P=0.001<0.05).ConclusionBoth Huacaishenlingbaizhu Powder combined with anti-VEGF and anti-VEGF alone can obtain good clinical efficacy in the treatme-nt of w AMD.At the same time,it was observed that the combination of traditional Chinese medicine and anti-VEGF treatment of w AMD based on the theory of“Gut–Retina Axis”had more significant e-fficacy than the single application of anti-VEGF treatment in te-rms of improving and stabilizing BCVA,reducing CMT,improving fundus exudation and alleviating TCM syndroms,which reflected t-he advantages of“combination of disease and syndrome”and compl-ementary TCM and western medicine.It is worth further discussion.
