| Objective:By observing the clinical effect of Jiedu fumigation and washing decoction to promote wound healing of diabetic foot after vascular intervention,the efficacy and safety of promoting wound healing was discussed.Methods:A total of 64 diabetic foot patients admitted to the department of peripheral vascular disease,Affiliated Hospital of Shandong University of Chinese Medicine from December 2021 to December 2022 were selected and divided into the treatment group(32 cases)and the control group(32 cases)by randomized control method.The control group was given routine and intravascular interventional therapy,and the treatment group was given detoxification and drug washing after intervention on the basis of the control group.The therapeutic effects were compared 24 hours before intervention,24 hours after surgery and 7 days after surgery,including wound area,wound pus,wound rotting flesh,wound granulation,White blood cell(WBC)count and C-reactive protein(CRP),Erythrocyte sedimentation rate(ESR),Ankle brachial index(ABI),and the effect of wound efficacy and TCM syndrome quantitative integration were evaluated to explore the efficacy and safety of promoting wound healing of diabetic foot.Results:1.In the course of this treatment,there were a total of 3cases of shedding,of which 1 was in the treatment group and 2 were in the control group.Finally,61 patients completed the clinical observation.2.Before treatment,there were no statistically significant differences in gender,age,diabetic foot course and glycosylated hemoglobin between the two groups(P>0.05),indicating comparability.3.On the 7th day after surgery,there was no statistical significance in the wound area of the treatment group compared with the control group(P>0.05).The wound healing rate between the two groups was compared,and the difference was statistically significant(P<0.05).4.On the 7th day after surgery,there were statistically significant differences between the treatment group and the control group in wound pus,wound carrion and wound granulation scores(P<0.05).5.On day 7 after surgery,there were statistically significant differences in WBC and CRP levels between the two groups(P<0.05);On the7 th day after surgery,the level of ESR between the two groups was compared,the difference was not statistically significant(P>0.05).6.The ABI index between the two groups was statistically significant on the 7th day after surgery(P<0.05);24 hours after surgery,ABI index of the two groups was statistically significant compared with 24 hours before surgery(P<0.05).7.In terms of wound efficacy and wound TCM syndrome quantized score efficacy,the total effective rate of the treatment group was 90.32% and93.54%,and the control group was 80.00% and 80.00%,both of which had significant statistical significance(P<0.05).8.There were no obvious abnormalities in the safety indexes of the two groups during medication.Conclusion:The blood flow of diabetic foot patients was significantly improved after endovascular interventional surgery.Postoperative combination of Jiedu fumigation and washing decoction was significantly superior to conventional dressing change combined with interventional surgery in promoting wound healing,relieving local symptoms and reducing inflammatory response,and was safe and effective,providing a new treatment approach of traditional Chinese and western medicine for promoting wound healing of diabetic foot. |
