Abstract/Summary:
ObjectiveThis study investigated the comfort level of patients who underwent radical gastrectomy before discharge.To understand the current comfort level of patients undergoing radical gastrectomy;To find out the population in need of key intervention and explore the influencing factors of comfort status of patients undergoing radical gastrectomy before discharge,in order to provide new ideas and intervention paths for scientific development of personalized nursing and improvement of comfort level of patients before discharge.MethodIn this study,132 patients who met the inclusion and exclusion criteria who underwent radical gastrectomy in the gastrointestinal surgery department of a third-class A hospital from December 2021 to August 2022 were selected by convenience sampling method.The General Information Questionnaire,Herth Hope Index(HHI)and Medical Coping Modes Questionnaire(MCMQ)were used on the day of admission.The Distress Management Screening Measure(DMSM),the simplified General Comfort Questionnaire,and the Distress Management Screening Measure(DMSM)were used 24 hours after surgery and 24 hours before discharge.The survey data were recorded in Excel by two people and then compared to ensure data quality.SPSS25.0 software was used for statistical analysis.Statistical methods included descriptive statistics,t test,one-way analysis of variance,Pearson correlation analysis,and multi-factor analysis.Result1.The total comfort score of patients undergoing radical gastrectomy was(81.13±7.76),and the incidence of significant psychological distress was 56.8%;The total score of comfort 24 hours before discharge was(89.55±7.25),and the incidence of psychological distress was 25.0%,which was better than that of comfort and psychological distress 24 hours after surgery.The total score of coping style was(40.62±1.66),and the score of facing,avoiding and yielding was(16.86±4.41),(16.71±3.67)and(7.05±1.34),respectively.The total score of hope level was(40.05±4.28),and the scores of realistic and future positive attitude(T),positive action(P),maintaining close relationship with others(I)were(13.02±1.63),(13.54±1.70)and(13.50±1.61)respectively.2.Univariate analysis showed that different age,education level and per capita monthly family income had statistically significant effects on the comfort status of patients undergoing radical gastrectomy 24 h before discharge(P<0.05).3.Correlation analysis shows that: Comfort status of patients undergoing radical gastrectomy 24 h after surgery was positively correlated with coping style and hope level(P<0.05)(0.312 and 0.204),and negatively correlated with avoidance,yield coping style and psychological distress(P<0.05)(-0.295,-0.342 and-0.447,respectively).Comfort status of patients undergoing radical gastrectomy 24 hours before discharge was positively correlated with hope level(r=0.284,P<0.05),and negatively correlated with psychological distress(r=-0.408,P<0.05).4.The results of multi-factor regression analysis showed that psychological distress,yielding coping style and hope level were the main influencing factors of postoperative comfort of gastric cancer patients 24h(P<0.05).Psychological distress before discharge,hope level,family per capita monthly income less than 2000 yuan,family per capita monthly income more than 5000 yuan were the main factors affecting the comfort status of patients undergoing radical gastrectomy 24 h before discharge(P<0.05).Conclusion1.The patients who underwent radical gastrectomy had a moderate level of comfort and discomfort before discharge.There is significant psychological distress;High level of hope;Mainly adopt the face type coping style.2.The comfort status of patients undergoing radical gastrectomy before discharge was related to hope level,coping style and psychological distress.The higher the level of hope,the lower the degree of psychological distress,the better the comfort.3.The comfort status of patients undergoing radical gastrectomy before discharge was mainly affected by per capita monthly income of family,yielding coping style,hope level and psychological distress.Medical staff should reasonably adjust medication plan to reduce economic burden;Encouraging positive responses;Strengthen psychological nursing intervention,reduce the degree of psychological distress,improve the level of hope,and gradually improve the comfort of patients.
