Objective Flow cytometry was used to detect the concentration of cytokines in the intraocular fluid of patients with clinically suspected tuberculous retinal vasculitis,and to explore the changes of cytokines in the intraocular fluid of patients with tuberculous retinal vasculitis,providing a new idea for the pathogenesis and diagnosis and treatment of tuberculous retinal vasculitis.Method Patients with retinal vasculitis were collected from May 2021 to June 2022 in Ophthalmology department of Subei People’s Hospital and divided into three groups.TRV group : clinical suspected tuberculous retinal vasculitis,13 patients with T cell spot test for tuberculous infection test(T-SPOT)positive,whose ocular manifestations were suspected to tuberculous retinal vasculitis and given anti-tuberculous treatment;p-RV group: T-SPOT positive idiopathic retinal vasculitis group,12 patients with T-SPOT positive and ocular manifestations inconsistent with tuberculous retinal vasculitis;n-RV group: T-SPOT negative idiopathic retinal vasculitis group,12 patients with idiopathic retinal vasculitis,whose serum infection and immune indicators were negative;group control: 14 patients with no other eye diseases except senile cataract were collected.Intraocular fluid was collected from each group at the first visit.The concentration of 12 cytokines(IL-1β,IL-2,IL-4,IL-5,IL-6,IL-8,IL-10,IL-12p70,IL-17,IFN-γ,TNF-α and IFN-α)in the intraocular fluid of patients in TRV group,p-RV group,n-RV group and control group was determined by flow cytometry.The differences in the concentration of cytokines in the intraocular fluid of patients in the above groups were compared and analyzed,and the distribution characteristics of cytokines in the intraocular fluid of patients with clinically suspected tuberculous retinal vasculitis were further analyzed,so as to explore the role of cytokines in the pathogenesis of tuberculous retinal vasculitis.Results1.Comparison of IL-6 concentration in intraocular fluid between the four groupsCompared with control group,the concentration of IL-6 in intraocular fluid of TRV group,P-RV group and n-RV group was significantly increased,with statistical significance(P=0.001,0.004,0.018).The concentration of IL-6 in intraocular fluid of TRV group was significantly higher than that of P-RV group and n-RV group,and the difference was statistically significant(P=0.040,0.010).2.Comparison of IL-8 concentration in intraocular fluid between the four groupsCompared with control group,the concentration of IL-8 in intraocular fluid of TRV group,P-RV group and n-RV group was significantly increased,with statistical significance(P=0.001,0.016,0.017).The concentration of IL-8 in intraocular fluid of TRV group was significantly higher than that of P-RV group and n-RV group,and the difference was statistically significant(P=0.036,0.035).3.Comparison of IL-1β concentration in intraocular fluid between the four groupsThe concentration of IL-1β in intraocular fluid of TRV group was lower than that of P-RV group,and the difference was statistically significant(P=0.028).The concentration of IL-1β in intraocular fluid of P-RV group was higher than that of n-RV group,and the difference was statistically significant(P=0.019).The concentration of IL-1β in intraocular fluid of P-RV group was higher than that of control group,and the difference was statistically significant(P=0.021).4.Comparison of IL-5 concentration in intraocular fluid between the four groupsThe concentration of IL-5 in the intraocular fluid of n-RV group was lower than that of control group(P=0.042),and the concentration of IL-5 in the intraocular fluid of P-RV group was higher than that of n-RV group(P=0.012).5.Comparison of IL-10 concentration in intraocular fluid between the four groupsThe concentration of IL-10 in intraocular fluid of TRV group was higher than that of control group,and the difference was statistically significant(P=0.038).6.Comparison of IL-17 concentration in intraocular fluid between the four groupsThe concentration of IL-17 in intraocular fluid of P-RV group was higher than that of control group,and the difference was statistically significant(P=0.007).7.Comparison of IL-4 and IL-12p70 concentrations in intraocular fluid between the four groupsThe concentrations of IL-4 and IL-12p70 in intraocular fluid of TRV group was lower than those of P-RV group,and the difference was statistically significant(P=0.033,0.013).Conclusion1.The concentrations of IL-6 and IL-8 in the intraocular fluid of clinically suspected tuberculous retinal vasculitis group were significantly higher than those of T-SPOT positive idiopathic retinal vasculitis group,T-SPOT negative idiopathic retinal vasculitis group and control group.The pathogenesis of tuberculous retinal vasculitis may be closely related to inflammatory cytokines such as IL-6 and IL-8.2.The concentrations of IL-1β and IL-5 in the intraocular fluid of T-SPOT positive idiopathic retinal vasculitis group were higher than those of T-SPOT negative idiopathic retinal vasculitis group.
