Objective:(1)Describe the level of fear of falling in hospitalized elderly patients with chronic heart failure.(2)To determine the influencing factors of fear of falling in hospitalized elderly patients with chronic heart failure.Methods: This study was a cross-sectional study in a non-experimental study,using the questionnaire survey method.From January 2022 to January 2023,convenience sampling was used to select elderly patients with chronic heart failure who met the inclusion and exclusion criteria in the Department of Cardiovascular Medicine(three wards)of Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine as the research objects.A general information questionnaire and the modified Survey of Activities and Fear of Falling in the Elderly were used.m SAFFE),FRAIL Scale,Charlson comorbidity index(CCI),short Geriatric Depression Scale(GDS-15),Social Support Rating Scale(SSRS).SSRS)were used to investigate the current situation of fear of falling in elderly patients with chronic heart failure.The relevant data were input by Epi Data3.0 software,and the data were statistically analyzed by SPSS25.0.Quantitative data were described by mean + standard deviation,median and quartile,and categorical data were described by frequency and percentage.Independent sample t test or approximate t test was used to compare the means of two groups of samples,one-way ANOVA or Welch test was used to compare the means of multiple groups of samples,and LSD or Tamheini method was used for pairwise comparison.Pearson correlation analysis was used to analyze the correlation between fear of falling and grip strength and up and go time(TUG).Multiple linear regression analysis was used to analyze the multiple factors of fear of falling.The test level α=0.05,P < 0.05 was considered statistically significant.Results:(1)A total of 198 hospitalized elderly patients with chronic heart failure were investigated in this study.The score of fear of falling was 33.24±8.24.(2)Univariate analysis showed that: The scores of fear of falling were correlated with age,emotional status,education level,medical payment method,social support level,duration of illness,cardiac function classification,number of medications,number of falls in the last year,whether someone fell around,assisted walking,comorbidity,frailty,depression level,grip strength level,Timed Up and Go test(Timed Up and Go)test,TUG),P<0.05.(3)Correlation analysis showed that There was a significant linear negative correlation between the overall grip strength level and the overall score of fear of falling in hospitalized elderly patients with chronic heart failure(r=-0.669,P < 0.001),and there was a significant linear negative correlation between the grip strength level and the score of fear of falling in male patients(r=-0.799,P<0.001).There was a significant linear negative correlation between grip strength level and fear of falling score in the female group(r=-0.824,P < 0.001).The overall TUG time in hospitalized elderly patients with chronic heart failure was significantly positively correlated with the overall score of fear of falling(r=0.829,P<0.001),and the TUG time in male group was significantly positively correlated with the overall score of fear of falling(r=0.838,P<0.001).There was a significant linear positive correlation between TUG time and the score of fear of falling in the female group(r=0.828,P<0.001).(4)Multivariate analysis showed that: TUGtime(β=0.218,P=0.001),age(β=0.382,P<0.001),social support(β=-0.152,P<0.001),history of falls(β=0.200,P<0.001),combined frailty(β=0.156,P<0.001).The above five influencing factors were statistically significant and entered the multiple linear regression equation.Conclusion(s):(1)The fear of falling in hospitalized elderly patients with chronic heart failure was at a moderate level.(2)The main influencing factors of fear of falling in hospitalized elderly patients with chronic heart failure include TUG time,age,history of falls,social support and combined frailty.(3)Health care professionals should pay attention to the assessment of the fear of falling in hospitalized elderly patients with chronic heart failure,focus on the decline in physical function and actively take intervention measures to promote fall prevention and maintain functional independence,so as to improve the level of fear of falling.
