Clinical Investigation And Analysis Of Patients With Pulmonary Nodules And Study Of TCM Syndrome Types

Abstract/Summary:
Objective: Through clinical collection of clinical data of patients with pulmonary nodules(diameter < 10mm),the correlation between the data of patients was further analyzed,and the distribution of main TCM syndromes of pulmonary nodules was summarized,which provided some reference for clinical prevention and treatment of pulmonary nodules.Methods: From October,2021 to December,2022,220 patients with small pulmonary nodules were collected from the outpatient department of Ruikang Hospital affiliated to guangxi university of chinese medicine.The clinical data of patients with small pulmonary nodules were collected,including basic information,tongue and pulse condition,common symptoms,average number of colds per year,eating habits,tobacco and alcohol history,mood,body fat rate and body mass index(BMI),so as to further analyze the patients’ data and lungs.Input the collected relevant information into Excel form,and finally analyze all the data with SPSS21.0 statistical software.Results: 1.A total of 220 patients with pulmonary nodules were included in this study,including 119 males,accounting for 54.09%.There are 101 females,accounting for 45.91%,the ratio of male to female is 1.17: 1,the average age of males is 56.46 15.26 years,the average age of females is 57.6614.95 years,and 195 cases are over 40 years old,and the majority of patients are middle-aged and elderly.2.After analyzing various data and nodule size,it was found that the nodule of patients with smoking history was larger than that of patients without smoking history,and the difference was statistically significant(P<0.05);The nodule size of patients with second-hand smoke history was significantly higher than that of patients without second-hand smoke history(P<0.05);There was a significant positive correlation between nodule size and age(r=0.168,P<0.05);There was no significant difference between the size of nodules and gender,diet,mood,smoking and alcohol history,gastroesophageal reflux history,and the history of "three highs"(P>0.05).3.After analyzing the data and the nature of nodules,it was found that the body fat rate of patients with solid nodules was significantly lower than that of ground glass and mixed nodules(P<0.05);The BMI of solid nodules was significantly lower than that of ground glass and mixed nodules(P<0.05);The solid and mixed nodules in patients with smoking history were significantly higher than those in ground-glass nodules(P<0.05);There was no significant difference between the nature of nodule and age,sex,diet,mood,second-hand smoke history,annual average number of colds,gastroesophageal reflux history,and history of three highs(P>0.05).4.220 patients with pulmonary nodules were included in this study.The common symptoms were: chills(56.36%),fatigue(50.45%),irritability(31.36%),hypochondriasis or pain(18.64%),chest tightness(15.45%),abdominal distension or pain(15.00%);Common tongue images include pale white tongue(47.73%),dark red tongue(19.09%),pale red tongue(18.64%),red tongue(18.64%),thin white fur(65.00%),and white greasy fur(12.70%);The common pulse patterns are: stringed pulse(67.27%),fine pulse(35.91%),weak pulse(15.00%),and several pulse(10.00%).5.220 patients with pulmonary nodules were included in this study.According to the analysis of their symptoms,physical signs and tongue pulse,a total of five syndrome types were summarized.Among them,59 cases(27%)were of yang deficiency with phlegm,followed by 54 cases(25%)of deficiency of qi and blood,48 cases(22%)of liver depression and qi stagnation,29 cases(13%)of phlegm and blood stasis,and 23 cases(10%)of spleen deficiency and damp-heat syndrome;After analyzing the irritability and TCM syndrome types,it was found that the patients with irritability and irritability had more liver-qi stagnation syndrome,with a significant difference(P<0.05);After analyzing the size of nodules of different syndrome types,it was found that the nodules of patients with phlegm and blood stasis syndrome were larger,with significant difference(P<0.05);There was no significant difference between TCM syndrome type and diet,nodule nature and gender(P>0.05).Conclusion:1.Smoking history,second-hand smoking history and age are all factors affecting the size of nodules.Patients with smoking history have more nodules than those without smoking history;The nodule size of patients with a history of second-hand smoke was significantly higher than that of patients without a history of second-hand smoke.2.Body fat rate,BMI and smoking history are all factors affecting the nature of nodules.Obese people are more likely to suffer from mixed and ground-glass nodules;The solid and mixed nodules of patients with smoking and alcohol history were significantly higher than those of ground glass nodules.3.The TCM syndrome types of patients with small pulmonary nodules include 59 cases of yang deficiency with phlegm and drink,54 cases of deficiency of qi and blood,48 cases of liver depression and qi stagnation,29 cases of phlegm and blood stasis,and 23 cases of spleen deficiency and damp-heat syndrome;Emotion and nodule size are the influencing factors of TCM syndrome type.The distribution proportion of phlegm and blood stasis syndrome increases with the increase of nodule.
